ObjectiveTo investigate the effect of pioglitazone alone combined with metformin on HbAlc,blood lipid and insulin sensitivity in elderly patients with type 2 diabetes mellitus,and to provide clinical reference and guidance for the treatment of elderly T2DM patients.Methods156 cases of elderly patients with type 2 diabetes mellitus outpatient and hospitalized patients were admitted in The People's Hospital of Wuhai Inner Mongolea from March 2014 to February 2015,after screening and exclusion,the remaining 120 cases,divided into the observation group and the control group,the control group was treated with pioglitazone alone,the observation group was treated with pioglitazone combined with metformin,to observe the levels of fasting blood glucose(FPG),2h postprandial blood glucose(2h PG),to measure glycosylated hemoglobin(HbAlc)and the levels of blood lipid,to measure fasting insulin(FINS)and postprandial 2h insulin(2h INS),to evaluate the insulin resistance index(HOMA-IR).Results1.The levels of FPG,2h PG and HbAlc after treatment in the two groups were significantly lower than those of before treatment(P<0.05),the levels of FPQ 2h PG and HbAlc after treatment in the observation group were significantly lower than those of the control group(P<0.05).2.The levels of TG,TC and LDL-C after treatment in the two groups were significantly lower than those of before treatment(P<0.05),the level of HDL-C was significantly higher than that of the control group(P<0.05);and the levels of TG,TC and LDL-C were significantly higher in the observation group than those of the control group(P<0.05),the level of HDL-C was significantly higher than that of the control group(P<0.05).3.The levels of FINS,2h INS and HOMA-IR after treatment in the two groups were significantly lower than those of before treatment(P<0.05),and the levels of FINS,2h INS and HOMA-IR after treatment in the observation group were significantly lower than that of the control group(P<0.05).4.The incidence of adverse reactions was 10%(6/60)in the observation group and 8.33%(5/60)in the control group,the incidence of adverse reactions in the observation group was slightly higher than that of the control group,but there was no significant difference between the two groups(P>0.05).Conclusion1.Pioglitazone combined with metformin can effectively reduce the blood glucose,blood lipid levels in the elderly patients with type 2 diabetes mellitus,and it can reduce insulin resistance,improve insulin sensitivity.2.Elderly patients with type 2 diabetes mellitus using pioglitazone combined with metformin is no serious adverse reactions,long-term use with good safety,it is worthy to clinical promotion.
