Purpose : To study the full-field flash electroretinogram(F-ERG)characteristics of Retinopathy of prematurity(ROP)in children with surgery and non surgery,to analyze the effects of ROP and laser surgery on retinal function.At the same time,animal experiments were used to observe the ERG characteristics of Oxygen induced retinopathy(OIR)rats and OIR rats after laser surgery.Furthermore,the effects of ROP and laser surgery on retinal function were analyzed from the point of view of animal experiments.MethodClinical experiment: Forty-six children(86 eyes)with with history of pre-threshold or threshold retinopathy of prematurity(ROP)aged Five months-3 years were recruited from the Department of Ophthalmology in Hunan Children's Hospital from January 2015 to December 2016.They were divided into the following two groups based on history of operation :(1)ROP operation group(22 cases,42 eyes)and(2)ROP non operationgroup(Because of the body or economic reasons without surgery,ROP degenerated spontaneously)(24 cases,44 eyes),and 19 cases(38 eyes)of premature infants without ROP were collected as control group.F-ERG examination was performed in each group,to analyze the changes of ERG parameters in each group,to evaluate the characteristics of retinal function,and to explore the influence of ROP and surgery on retinal function,which had been excluded from the F-ERG examination or accompanied by other diseases and severe systemic diseases.Animal experiment: Twenty-four neonatal Sprague Dawley rats were divided into OIR group and control group.In OIR group,12 rats were exposed to(75±2)% oxygen for 7 days and then exposed to room air for 7 days;in control group,12 rats were raised in room air for 14 days.After the success of modeling,the rats at the age of 21 days,the remaining 10 rats in group OIR and 10 rats in control group were examined by F-ERG,recording the rod response,the maximum mixing reaction and the cone reaction.Then 10 OIR rats underwent laser surgery,and after 1 weeks,9 OIR rats(one died)underwent F-ERG examination again.The ERG results of OIR rats before and after operation were matched and compared.ResultsClinical experiment results:ROP operation group,ROP non operation group and control group comparison.(1)Compared with the other two group,the b-wave amplitudes decreased(F=7.98,P=0.001)and the implicit times increased(F=15.74,P=0.000)in rod response in ROP operation group,the differences were statistically significant(P<0.05);(2)the a and b-wave amplitudes decreased(F=7.931,P=0.001;F=8.025,P=0.001)and the implicit times increased(F=9.393,P=0.000;F=8.953,P=0.000)in the maximum mixing reaction in ROP operation group,the differences were statistically significant(P<0.05).(3)There was no difference between ROP operation group,ROP non operation group and control group on a-and b-wave amplitudes(F=0.318,P=0.728;F=0.517,P=0.598)and implicit times(F=1.027,P=0.361;F=0.781,P=0.460)in cone response(P>0.05).Animal experiment results: OIR group and control group comparison.(1)Compared with the control group,the b-wave amplitudes decreased(t=3.650,P=0.001)and the implicit times increased(t=2.410,P=0.021)in rod response in OIR group,the differences were statistically significant(P<0.05);(2)the a and b-wave amplitudes decreased(t=3.333,P=0.002;t=2.562,P=0.016)and the implicit times increased(t=2.725,P=0.010;t=2.482,P=0.020)in the maximum mixing reactionin OIR group,the differences were statistically significant(P<0.05).(3)There was no difference between OIR group and control group on a-and b-wave amplitudes(t=0.650,P=0.520;t=0.204,P=0.809)and implicit times(t=0.422,P=0.600;t=0.076,P=0.940)in cone response(P>0.05).Comparison of pre-operative and post-operative self matching in OIR rats.(1)Compared with the pre-operative OIR group,the b-wave amplitudes decreased(t=3.019,P=0.008)and the implicit times increased(t=3.893,P=0.001)in rod response in pre-operative OIR group,the differences were statistically significant(P<0.05);(2)the a and b-wave amplitudes decreased(t=8.056,P=0.000;t=2.668,P=0.016)and the implicit times increased(t=3.298,P=0.004;t=7.986,P=0.000)in the maximum mixing reaction in pre-operative OIR group,the differences were statistically significant(P<0.05).(3)There was no difference between pre-operative and post-operative OIR group on a-and b-wave amplitudes(t=1.291,P=0.214;t=0.639,P=0.531)and implicit times(t=1.144,P=0.269;t=1.122,P=0.278)in cone response(P>0.05).Conclusion1.The main effect of ROP on retinal function was the decrease of the functional response of rod cells,but little effect on the cones.2.Compared with ROP,laser surgery has a greater impact on the retinal function,which is mainly manifested in the decline of the function of the rod cells,while the effect of the cone cells is less.
