Background With the development of our economy,the way of life and the change of diet structure,the incidence of colorectal cancer is increasing year by year.The colorectal is divided into different pathological types.Various pathological and pathological colorectal cancers have different clinicopathological features and survival prognosis.Colorectal mucinous adenocarcinoma and signet ring cell carcinoma are a special type of colorectal cancer.Regardless of the age of onset,location,or survival,they are quite different from those of general pathology.The incidence of colorectal mucous adenocarcinoma and signet ring cell carcinoma is low.Summary data help to further discover its own specificity.CEA and CA199 as tumor markers,were widely used in gastrointestinal cancer clinical diagnosis,prognosis and postoperative colorectal cancer with different properties of CEA and CA199.The sensitivity and specificity of CEA were different,and the level of CA199 in colorectal cancer and clinical pathology and prognosis are closely linkedObjective To investigate the relationship between preoperative serum CEA and CA199 levels and prognosis of colorectal mucinous and signet ring cell carcinoma,and to evaluate the feasibility of CA199 and CEA levels in patients with colorectal mucinous and signet ring cell carcinoma.Method The clinicopathological and follow-up data of 446 cases of colorectal mucinous cancer and signet ring cell carcinoma at the First Affiliated Hospital of Zhengzhou University from July 2008 to November 2014 were selected to analyze the preoperative CEA,CA99 levels and the clinicopathological factors of the patients,as well as the relationship with the survival prognosis of the patients.The relationship between preoperative serum CEA and CA199 levels and clinicopathological features of colorectal mucinous adenocarcinoma and signet ring cell carcinoma was analyzed by chi square test.2.K-M method was used to draw survival curves and Log Rak method was used to compare survival differences.COX risk proportional model was used to analyze univariate and multivariate factors that might affect the survival and prognosis of patients.Result The results of single factor analysis showed that preoperative serum CEA level and age,neutrophil count,tumor location,tumor size,TNM stage(P<0.05),and has nothing to do with the patient’s family history,there is no nerve complications,vascular invasion,pathological type(P>0.05);tumor location,tumor the serum level of CA199 before the operation,the size of the TNM stage(P<0.05),but not related with age,neutrophil count,family history,complication,nerve vascular invasion,pathological type(P>0.05).The survival curves of K-M showed that there was great difference in survival between patients with signet ring cell carcinoma and mucinous adenocarcinoma.The survival rate of patients with signet ring cell carcinoma was significantly shorter than that of patients with signet ring cell carcinoma(P=0.000).The survival rate of mucinous adenocarcinoma of different levels of CEA and CA199 group was statistically significant(P=0.008),while the difference of CEA and CA199 level between India and ring cell carcinoma patients was not statistically significant(P=0.062).In the multivariate survival analysis of COX,right half colon cancer(P=0.038),TNM stage late(P=0.000),signet ring cell carcinoma(P=0.015)and preoperative CA199 level(P=0.002)were independent risk factors affecting the overall survival of patients.Conclusion The preoperative serum CEA and CA199 can be used as a reference index for predicting the prognosis of colorectal mucinous adenocarcinoma and signet ring cell carcinoma,and it is feasible. |
