ObjectiveThe patients with raised erosive gastritis(REG)were examined by magnifying gastroscopy.HE staining and p16 protein detection were performed on the mucosal tissues of targeted biopsy guided by magnified gastroscopy,and the detectable rates of different pathological manifestations of different expression of p16 protein were analyzed.To investigate the significance of p16 protein combined with magnifying gastroscopy in screening early gastric cancer(EGC)and precancerous lesions in patients with REG.MethodsFrom February 2017 to December 2017.100 patients who examined by endoscopy and met the diagnostic criteria of REG in the outpatient department of our Hospital were selected as study subjects.The patients with REG were examined by magnification gastroscopy.The p16 protein was detected by HE staining and immunohistochemical staining SP method.According to the expression level of p16 protein,it was divided into p16 positive group and p16 negative group.The pathological findings of the two groups were analyzed.The detection rates of EGC and precancerous lesions were compared between the two groups.Results1.There were 22 cases of atrophy(22%),26 cases of intestinal metaplasia(26%),9 cases of low-level epithelial neoplasia(9%),57 cases of precancerous lesions(57%)and 4 cases of high-grade epithelial neoplasia(4%)in REG patients.2.There were 52 patients of p16(+)and 48 cases of p16(-)in REG patients.The incidence rate of atrophy was 26.9%(14/52)and 16.7%(8/48)respectively.The rate of intestinal metaplasia was 17.3%(9/52)and 35.4%(17/48),and the rate of low-grade epithelial neoplasia was 1.9%(1/52)and 16.7%(17/48).The atrophy was not statistically significant(P>0.05).However.intestinal metaplasia and low-grade epithelial neoplasia were statistically significant(P<0.05).3.4 cases of early gastric cancer were in p16(-)group(4/48,8.3%),while the pl6(+)group had no early gastric cancer(0/52.0%),but the difference was not statistically significant(OP>0.05).4.The rate of precancerous lesions in p16(+)group and p16(-)group was 46.2%(24/52)and 68.8%(33/48),and the difference was statistically significant(P<0.05).The rate of precancerous lesions and early gastric cancer was 46.2%(24/52)and 77.1%(37/48),and the difference was statistically significant(P<0.05).ConclusionThe detection rate of precancerous lesions and early gastric cancer can be increased by magnified gastroscope combined with p16 protein detection of in REG patients with targeted biopsy under the guidance of ME-NBI,which may be a method for screening early gastric cancer and precancerous lesions in REG patients.
