Clinical Research Of Supplemented Tianma Gouteng Decoction In Patients With Morning Hypertension Induced By Hyperactivity Of The Liver-yang

Objective:The objectives of the trial are to observe clinical effect of Supplemented Tianma Gouteng Decoction in patients with morning hypertension induced by hyperactivity of the liver-yang.Methods:A total of 40 patients with morning hypertension induced by hyperactivity of the liver-yang were randomly divided into treatment group and control group,with 20 cases in each group.Both groups were given conventional anti-hypertensive therapy of western medicine,and the treatment group was treated with Supplemented Tianma Gouteng Decoction for one dose per day,the treatment of both groups was lasted for 4 weeks.Indicators such as morning blood pressure,mean blood pressure,standard deviation of blood pressure and scores of TCM syndrome score of both groups were observed and analyzed before and after treatment,and evaluate the effect of Supplemented Tianma Gouteng Decoction was evaluated.Results:1.Morning blood pressure:the control group and treatment group had decreased morning blood pressure after 4-week treatment than treatment before(P<0.05 or P<0.01),and the treatment group decreased more significantly than the control group(P<0.05).The treatment group had a more significant reduction in morning diastolic blood pressure(DBP)after treatment than the control group,but there was no significant decline in morning systolic blood pressure(SBP)between groups(P>0.05).After treatment,standard-reaching rate of morning blood pressure was 75%in the treatment group,and 45%in the control group,but no significant difference was seen in two groups(P>0.05).2.Mean blood pressure:mean blood pressures in each period were decreased in two groups after treatment than treatment before(P<0.01).The treatment group had more significant reduction in mean SBP levels in each period than the control group(P<0.05),but mean DBP levels in each period and decrease degree of the mean blood pressure showed no significant between-group significance(P>0.05).3.Blood pressure variability(BPV):after treatment,standard deviation of blood pressures in each period in two groups were decreased in varying degrees(P<0.05 or P<0.01),and treatment group had better outcomes in reduction of 24-hour systolic blood pressure standard deviation(24 h SSD),24-hour diastolic blood pressure standard deviation(24 h DSD),and day diastolic blood pressure standard deviation(dDSD)than the control group(P<0.05),but the decrease degree of standard deviation of standard deviation of blood pressures in each period showed no significant differences in two groups(P>0.05).4.Circadian rhythm of blood pressure:the both groups separately had 5 patients with dipper type hypertension before treatment,and the treatment group increased to 7 cases,but the control group presented no significant increase,and no significant difference was observed in two groups(P>0.05).5.TCM syndrome scores:the total scores of TCM syndrome in both groups after treatment was significantly lower than treatment before(P<0.01),and the overall improvement in the treatment group was superior to that in the control group(P<0.01).Except for items of TCM syndrome such as red eyes,soreness of waist and soft knee,the other items of TCM syndrome after treatment had varying improvement in the treatment group(P<0.05 or P<0.01),which was significantly superior to the control group(P<0.05 or P<0.01).The total effective rate of TCM syndrome in the treatment group and control group were 90%,and 45%,respectively,which showed a significant difference in two groups(P<0.01).Conclusions:Chinese compound formula named Supplemented Tianma Gouteng Decoction in combination with anti-hypertensive therapy of western medicine can effectively reduce the level of morning blood pressure and significantly improve clinical symptoms related to hyperactivity of the liver-yang,and enhance control rate of morning blood pressure,and life quality of patients with morning hypertension induced by hyperactivity of liver-yang,and this study demonstrates that its clinical efficacy is superior to the mono-therapy of western medicine.Meanwhile,the combination of Supplemented Tianma Gouteng Decoction and anti-hypertensive drugs are able to reduce the mean blood pressure in each period,and improve the BPV in some.extent,but have unobvious impact on recovery of normal blood pressure circadian rhythm.
