Clinical Observation On Hp-associated Chronic Gastritis With Heat-wet In Spleen And Stomach In The Treatment Of Modified "Banxia Xiexin Decoction" Combined With Quadruple Therapy

ObjectiveTo observe the clinical effect of Hp-associated Chronic Gastritis with heat-wet in spleen and stomach in the treatment of modified“Banxia Xiexin Decoction”combined with quadruple therapy,and to analyse the feasibility and security,to provide a reliable basis for Chinese medicine treatment of H.pylori related chronic gastritis.Methods72 patients who visited the doctor in department of Traditional Chinese Medicine of Wuhan integrated traditional Chinese and Western Medicine Hospital from January 2016 to January 2017,were diagnosed with Hp-associated chronic gastritis with damp-heat pattern of syndrome of spleen and stomach.They were randomly divided into observation group(36cases)and control group(36cases).There were five cases shedding in the observation group,and 31 cases completed.In the control group,three cases fell off and finally completed 33 cases.The control group was given quadruple therapy anti-Hp of western medicine Omeprazole Enteric-coated Tablets(20mg bid),Colloidal Bismuth Pectin Capsules(0.1g qid),Amoxicillin Capsules(1g bid)and Clarithromycin Tablets(0.5g bid),the drug was stopped after half a month.The observation group was given modified“Banxia Xiexin Decoction”(RhizomaPinellinaePraeparata10g,Coptis chinensis6g,BaikalSkullcap10g,Ginger3g,ageratum10g,Radix Pseudostellariae15g,dandelion30g,liquorice6g,Jujube6g),on the basis of the treatment of the control group.Usage:Take one dose daily,200m1 every time,half an hour after breakfast and dinner.The western drugs were stopped after half a month,and the Chinese medicine was stopped after one month.And then,the general data of the two groups,the rate of Hp eradication,the curative effect of TCM syndrome,the score of TCM symptoms,the score of the quality of life,the recurrence rate of Hp and the occurrence of adverse events were observed.Result1.The general data of two groups were compared:There was no significant difference in gender,age,course of disease and type of gastritis between the two groups(P>0.05).2.The rate of Hp eradication in the two groups was compared:The rate of HP eradication in the observation group after treatment(93.5%)was higher than that of the control group(78.8%),and the difference was not statistically significant(?~2=2.89,P>0.05).3.Comparison of curative effect of TCM syndrome in two groups:The effective rate of the observation group(93.5%)was higher than that of the control group(69.7%),and the difference was statistically significant(?~2=7.9,P<0.05).4.Comparison of TCM symptom scores before and after treatment in two groups:Before treatment,there was no significant difference in TCM symptom scores between the two groups(t=0.268,P>0.05).After treatment,the comparison in the group:the t values of the observation group and the control group were 16.268 and 17.082respectively,the differences were statistically significant(P<0.01);and the observation group after treatment of TCM symptoms was significantly lower than that of the control group,the difference was statistically significant(t=8.426,P<0.01).5.Comparison of the quality of life before and after treatment in two groups:Before treatment,there was no significant difference between two groups of the patients'core symptoms,physiological state,mental state,social activity,and digestive system diseases(P>0.05).After treatment,the comparison in the group:the scores of before and after treatment of observation group were compared,the difference was statistically significant(P<0.05);the control group was statistically significant(P<0.05)in the core symptoms,and the other four scores was not statistically significant(P>0.05);The difference between the two groups in the core symptoms was not statistically significant(P>0.05),and the other four scores in the observation group were higher than those of control group,the difference was statistically significant(P<0.05);6.Comparison of the recurrence rate of Hp in the two groups after treatment:The patients were followed up with 29 cases in the observation group and 26 cases in the control group.After six months,the recurrence rate of Hp in the observation group and the control group was 6.9%and 26.9%,respectively.The difference was statistically significant(P<0.05).After one year,the recurrence rates of the observation group and the control group was 17.2%and42.3%,respectively,and the difference was statistically significant(P<0.05).7.The incidence of side effects in the two groups:In the observation group,there were two cases of side effects,one case of vomiting and one case of diarrhea.In the control group,there were seven cases of side effects,two cases of dizziness,one case of vomiting,three cases of diarrhea,one case of constipation.The side effects of these patients were mild,and they did not reach the standard of falling off.In the course of the study,the patients were improved or cured after treatment.ConclusionThe effect of modified“Banxia Xiexin Decoction”and quadruple therapy anti-Hp in the treatment of Hp related chronic gastritis is better than simple quadruple therapy.It can obviously improve the clinical symptoms of the patients,reduce the recurrence rate of Hp,improve the quality of life,and have the advantages of high safety and small adverse reactions.It is worthy of clinical application.
