Objective: To investigate the effect of local combined use of rh GM-CSF and insulin on the healing of scalded wound in type 1 diabetic rats,and to explore the related mechanisms.Methods: In the animal laboratory,60 healthy male SD rats were fed with conventional feed for 1 week.After the serial number,50 SD male rats were extracted and the diabetic model was produced.After the fasting of 12 h,the rats were given a single dose of the 1% of the urine of STZ solution,according to the weight of the rats,55 mg/kg.A single dose of intraperitoneal injection(STZ)was given to the 1% of the fresh preparation,and A group was injected with the same dose of citric acid buffer.After 3 days,the blood glucose was measured(the venous blood of the rat tail),and 1%STZ peritoneal injection(10 ~ 15 mg/kg)was once again used in the fresh preparation.After the rats' blood sugar stabilized(after injection for 3 to 4 weeks),the rats on both sides of the rats caused a deep 2 degree burn wound.After the success of the diabetic scald model(10 failed),the number of rats was randomly divided into five groups including diabetic scald control group(B),insulin group(C),rh GM – SCF group(D),insulin combined rh GM – SCF treatment group(E),the remaining 10 rats were normal scald control group(A).the wound surface of group A was covered with gauze treated with 5 m L physiological saline.The wound surface of group B was treated with gauze with 5 m L physiological saline.The wound surface of group C was treated with gauze of 5 U and 30R+5 m L saline.Group D wound covering was treated with 25 ug rh GM-CSF+5 m L of saline.Group E was covered with gauze covering 5U and 30R+25 ug rh GM-CSF+5 m L physiological saline.In 3,7,11,15,21 days,the naked eye see groups of wound healing,and wound healing rate.CD31 HIF-1? and VEGF positive expression were detected by immunohistochemistry.Fluorescence PCR was used to measure the content of HIF-1? m RNA.Results: The histological observation of pancreatic tissue in diabetic rats was conducted randomly,and all of the cells in the islet tissue were destroyed.That is to induce successful diabetes mellitus.HE stained pathological proves: randomly selected groups of deep burn wounds with full-thickness skin is ?degree burn wounds?In 3,7,11,15,21 days,each group of rats was gradually reduced or healed,and the healing rate gradually increased.In group A and group E,fibroblasts and newborn capillaries were the fastest growing and the fastest healing.The wound healing rate was significantly higher in group A,C,D and E after scald than in group B(p <0.01),which was statistically significant.The wound healing rate of group A and group E was higher than that of group C and D(p <0.05),which was statistically significant.Comparison of group A and group E,group C and D(p >0 0.05),no statistical significance.The expression of VEGF positive expression in each group was expressed,its concentration was low,then increased,reached the peak,and then gradually decreased.3,7,11,15 and 21 days after scald,the VEGF positive expression of group A,group C,group D and group E was higher than group B(p <0.05),which was statistically significant.The VEGF positive expression in group A and group E was higher than group C and group D(p <0.05),which was statistically significant.The positive expression of HIF-1? was expressed after the injury,the concentration was low,then increased,reached the peak,and then gradually decreased.In the third,7th,11 th,15th and 21 days,the positive expression of HIF-1? in group A,C,D and E was significantly lower than that in group B(p <0.01),with significant statistical significance.In the third,7th and 11 th days after scald,the positive expression of HIF-1? in group C and group D was higher than that in group A and group E(p <0.05),with significant statistical significance.The relative amount of HIF-1? m RNA was low,then increased,reached the peak,and then gradually decreased.In the 7th,11 th,15th and 21 st days,the relative amount of HIF-1? m RNA in group E was lower than that of group A,B,C and D(p <0.01),with significant statistical significance.The positive expression of HIF-1? in group C and group D was lower than group B(p <0.01),which was statistically significant.The comparison between group C and group D(P<0.05)was different.The positive expression of CD31 began to be low,then increased,reached the peak,and then gradually decreased.In the 3rd,7th,11 th,15th and 21 st days,the positive expression of CD31 in group A and E was higher than that of group B,C and D(p <0.05),which was statistically significant.In the third,7th and 21 st days after scald,the positive expression of CD31 in group C and D was higher than that in group B(p <0.05),which was statistically significant.In the 11 th and 15 th days,there was no significant difference between group C and group B.Conclusion: local application of combined with insulin rh GM –CSF accelerate the diabetes burn wound healing,which may correct wound local oxygen,improve the flow of blood to the wound,promote the scalded wound the micrangium regeneration,and promote the healing of wounds.The mechanism may be related to inhibiting the expression of HIF-1? and promoting VEGF expression.
