BackgroundBreast cancer is one of the most common malignancies in women,the incidence rate is on the rise.It has been reported in the literature,it has a higher mortality rate in female diseases,there are second in the world,it has a serious impact on women's health,therefore,early detection of breast cancer is critical.Data show,early detection and early treatment of breast cancer can reduce the mortality rate by 40%.In the diagnosis of breast cancer,It is very important to choose the non-invasive,convenient and feasible method,which can be used repeatedly and has high sensitivity.At present,imaging examination is an important method for the diagnosis of breast diseases,among them,ultrasound and mammography(molybdenum X-ray photography,MX-r P)are the most commonly used methods of clinical examination.Conventional ultrasonography has a better advantage in detecting breast tumors.However,there are still limitations in qualitative identification of breast masses,such as color Doppler ultrasound,ultrasonic firefly technology,spectrum Doppler,gray scale ultrasound,and so on,can overlap partially.molybdenum target X ray imaging has some shortcomings in the diagnosis and differential diagnosis of tumor,it has radioactive hazards,In histology,the contrast is poor.But there are studies showing that,this method has high accuracy in detecting>2cm mass.This study is based on the criteria of surgical pathology,60 cases of breast masses examined and operated in Puyang Maternal and Child Health-Care Hospital are collected,after routine ultrasound,They are switched to ultrasound elastography and molybdenum target X-ray examination,the two methods are used to evaluate the differential diagnosis of benign and malignant breast tumors,malignant tumors of different sizes,and ductal carcinoma in situ of the breast.ObjectivesTo compare the value of ultrasound elastography and mammography in the diagnosis of breast cancer,to explore the advantages and disadvantages of these two methods in the diagnosis of breast cancer,to provide more scientific and accurate diagnostic methods for clinical breast cancer diagnosis.Methods60 cases of breast masses confirmed by surgery and pathology in our hospital from January 2015 to October 2016 were selected,all patients were female,there were 98lesions.All patients were examined by routine ultrasonography before operation.The characteristics of two-dimensional ultrasonography,the size and blood flow of the tumor were recorded,elastic imaging scores were then performed.5 point method was used for evaluation.The results of elastography were statistically analyzed,the lesions of benign and malignant lesions were evaluated according to the score.The X-ray examination of molybdenum target was usually oblique and biaxial,when necessary,the film was locally pressurized,the detection of benign and malignant lesions by molybdenum target X-ray was analyzed.The sensitivity and specificity of the two methods were compared and analyzed.The 98 breast lesions were divided into 3 groups according to the length of the long axis measured by ultrasonography:diameter?1.0cm was A group,diameter in1.1cm~2.0cm was B group,diameter>2.0cm was C group.The specificity and sensitivity of each group using two methods were compared.Finally,the significance of the two methods in the diagnosis of ductal carcinoma of the breast was analyzed.Results1.In 98 lesions,pathological findings showed,there were 35 malignant lesions,there were 63 benign lesions.In 35 malignant lesions,the leakage error rate was 20%by elastography alone,the X-ray leak detection rate of molybdenum target was 22.86%,the combined detection error rate was 2.85%,individual detection and joint detection were compared,the differences were statistically significant(P<0.05).In 63 cases of benign lesions,the leakage error rate was 20.63%by elastography alone,the X-ray leak detection rate of molybdenum target was 23.81%,the combined detection error rate was6.35%,individual detection and joint detection were compared,the differences were statistically significant(P<0.05).The specificity and sensitivity of combined detection were higher than that of single method,P<0.05.2.Group A(diameter?1cm)elasticity imaging sensitivity was 80%,the specificity was 84.21%,it was higher than molybdenum target X-ray(sensitivity was 60%,the specificity was 63.16%),but it was checked by x~2,there was no universal significance in difference(P>0.05).The sensitivity and specificity of joint detection were better than that of individual detection,but it was checked by x~2,there was no universal significance in difference(P>0.05).The sensitivity of elastography in group B(diameter 1.1cm~2.0cm)was 75%,the specificity was 75%,compared with mammography,the difference was not statistically significant,P>0.05;the sensitivity and specificity of combined detection were 91.67%and90%respectively,compared with elastic imaging,mammography,the sensitivity and specificity were significantly increased,but the difference was not statistically significant(P>0.05).The sensitivity of C group(diameter>2.0cm)was 76.92%,the specificity was 79.17%,compared with mammography,the difference was not statistically significant(P>0.05).the sensitivity of combined detection was 100%,the specificity was 95.83%,the sensitivity and specificity of combined detection were significantly higher than that of elastic imaging,molybdenum target X-ray,but it was checked by x~2,the difference was not statistically significant(P>0.05).3.In the 13 intraductal carcinomas,the detection rate of ultrasound elastography was84.62%,its detection rate was higher than that of mammography(69.23%),but it was checked by x~2,the difference was not statistically significant(P>0.05),the detection rate of combined examination was 100%,the detection rate was significantly higher than that of single examination,but it was checked by x~2,P>0.05.Conclusion1.Ultrasound elastography and mammography have certain advantages in the diagnosis of breast cancer,joint detection can be complementary to each other,it can improve the diagnostic accuracy of breast cancer,reduce the leakage rate.2.Compare with molybdenum target X-ray,ultrasound elastography has some advantages in the diagnosis of small breast lesions(diameter?1.0cm);ultrasound elastography combined with mammography can improve the accuracy of the diagnosis of breast cancer of different sizes.3.Ultrasound elastography has some advantages in the diagnosis of ductal carcinoma in the breast,the combination of mammography and X-ray examination can improve the diagnostic accuracy.
