Objective: To investigate the expression and clinical significance of PD-1/PD-L1 and its related cytokines(IL-2,IL-10)in gastric cancer and lymph node tissue.Assess its effect on tumor progression of gastric cancer,so as to provide more basis the prevention,diagnosis and treatment of gastric cancer.1.To understand the expression of PD-1/PD-L1 and IL-2/IL-10 in gastric cancer and lymph node tissue;2.To explore the correlation between PD-L1 and PD-1,IL-2,and IL-10,and their relationship with clinicopathological features,In order to provide more theoretical basis for the diagnosis and immunotherapy of gastric cancer,the relationship between them and the occurrence and development of gastric cancer is discussed.Methods: Forty cases of gastric cancer tissue and third groups seventy-four cases of lymph node tissue were detected by immunohistochemical method by SP,LN(+)indicates positive metastatic lymph nodes in patients with lymph node metastasis,LN(-)indicates non metastatic lymph nodes in patients with lymph node metastasis,and LN indicates lymph nodes in patients with no lymph node metastasis..Analysis using SPSS21.0(SPSS,US)statistical software,The expression of PD-1/PD-L1 and IL-2/IL-10 and the correlation analysis with various clinicopathological features were analyzed by chi 2 test,The correlation between the expression of PD-L1 and PD-1,IL-2 and IL-10 using Spearman rank correlation analysis,P<0.05 indicated that the difference was statistically significant.Results:1.The positive expression of PD-1 was manifested mainly in the cytoplasm and the membrane of the infiltrating lymphocytes of the gastric carcinoma,and the light yellow to brown granules appeared on the membrane,The positive expression rate in gastric cancer tissues was 40%(16/40),the positive rate of LN(+)expression was 70.6%(24/34),the positive rate of LN(-)was 38.2%(13/34),and the positive rate of LN expression was 33.3%(2/6),the difference was statistically significant(P < 0.05).The positive expression of PD-L1 was mainly manifested in the cytoplasm and the membrane of the infiltrating lymphocytes of the gastric cancer cells and the carcinoma cells,and the light yellow to brown granules appeared on the cell membrane.The positive rate of expression on gastric cancer cells was 55%(22/40),while the positive rate of expression on infiltrating lymphocytes was 37.5%(15/40),the positive rate of LN(+)expression was 64.7%(22/34),the positive rate of LN(-)was 32.4%(11/34),and the positive rate of LN expression was 16.7%(1/6),the difference was statistically significant(P < 0.05).2.The positive expression of IL-2 and IL-10 is mainly manifested in gastric cancer cells and some interstitial cells,and the light yellow to brown particles appear on the cytoplasm and cell membrane.The positive rate of IL-2 expression in gastric cancer tissues was 37.5%(15/40),the positive rate of LN(+)expression was 29.4%(10/34),the positive rate of LN(-)was 64.7%(22/34),and the positive rate of LN expression was 66.7%(4/6),the difference was statistically significant(P < 0.05).The positive rate of IL-10 expression in gastric cancer tissues was 47.5%(19/40),the positive rate of LN(+)expression was 67.6%(23/34),the positive rate of LN(-)was 35.3%(12/34),and the positive rate of LN expression was 50.0%(3/6),the difference was statistically significant(P < 0.05).3.The positive expression of PD-L1 in gastric cancer tissues was significantly correlated with TNM stage,depth of invasion and lymph node metastasis(P < 0.05),but not correlated with other clinical and pathological characteristics such as sex,age,tumor location and preoperative CEA level(P > 0.05).There was no significant correlation between the positive expression of PD-1 in gastric carcinoma and the clinicopathological features(P > 0.05).4.The positive expression of IL-2 in gastric carcinoma was correlated with TNM stage,depth of invasion,lymph node metastasis(P < 0.05),and had no significant correlation with other clinicopathological characteristics(P > 0.05).The positive expression of IL-10 in gastric cancer tissues was significantly correlated with TNM stage,differentiation degree and lymph node metastasis(P < 0.05),but not correlated with other clinicopathological features(P > 0.05).5.There was no significant correlation between the expression of PD-L1 protein and PD-1 expression in gastric cancer tissues(P > 0.05),which was negatively correlated with the expression of IL-2(r=-0.337,P < 0.05),and positively correlated with the expression of IL-10(r= 0.357,P < 0.05).Conclusion:1.PD-1,PD-L1 and IL-10 were highly expressed in gastric cancer and metastatic lymph nodes,but the expression of IL-2 is the opposite.The expression level of PD-L1 and IL-2 is closely related to TNM stage,depth of tumor invasion and lymph node metastasis.The expression of IL-10 was significantly correlated with TNM stage,differentiation degree and lymph node metastasis.2.There was no significant correlation between the expression of PD-L1 protein and the expression of PD-1 in gastric carcinoma,which was negatively correlated with the expression of IL-2 and was positively correlated with the expression of IL-10.3.The difference in expression of PD-1/PD-L1 and IL-2/IL-10 in gastric cancer and lymph nodes and their relationship with clinicopathological features may indicate that PD-1/PD-L1 may be involved in the development of gastric cancer and lymph node metastasis,It may be one of the factors predicting the progression of gastric cancer and affecting the prognosis.
