With the increasing proportion of the elderly population,the problem of population aging has become increasingly prominent.The demand and requirements of medical services for the special groups of the elderly are high,which are the focus of medical insurance and medical support services.China's basic medical insurance system has been established since the end of 20th century.In recent years,the reform and innovation of the medical insurance system is unprecedented.However,due to many social,economic and historical reasons,China's medical insurance system still exist many problems.The study of the impact of China's medical insurance system to reduce the economic burden of the elderly have a profound practical significance.After reviewing the previous literature,this paper first introduces the aging population of China and the status quo of the health of the elderly.With the analysis of development of medical insurance,we elaborated theoretical basis on this study combined with relevant economic theory and medical insurance theory.Secondly,based on the calculation method of medical burden,we choose macro-cost method and human capital method to measure direct medical expenses and indirect medical expenses separately,and to determine other relevant indexes of medical economic burden.In addition,this paper selects the CHARLS2013 national baseline survey data and use appropriate empirical model to control endogeneity and sample selection bias problem,then measure the impact of different medical insurance system on various types of medical economic burden indicators.Also,it distinguishes the difference between outpatient and inpatient,urban and rural areas and different age group.Finally,in order to make a prediction of the effect of health insurance and critical illness insurance on catastrophic health insurance expenditure,this paper makes a further study.It can be seen from the empirical analysis that the effect of the basic medical insurance on the inpatient costs is more significant than the outpatient costs,in which the urban workers' medical insurance significantly increases the outpatient and inpatient costs.Benefit from medical resources and economic conditions,Urban residents' cost of medical expenses is far greater than that of rural areas.And the expenditure on medical expenses of different age groups is also significantly different.The medical insurance significantly reduces the proportion of self-supporting medical expenses,but the role of reducing the burden of medical self-care is relatively weak.Also,the effect of different income groups is different,and the effect on low-income group needs to be strengthened.This paper improves the indicators of medical burden evaluation,and make a more comprehensive analysis of the effect of medical insurance system on medical economic burden.At the same time,this paper forecasts the effect of the serious illness insurance on catastrophic health expenditure.It provides a reference for policy implementation in the future.
