The Case Management Study On The Living Adaption Of Oldest Old In Nursing Home

With the rapid development of the social economy in our country,medical technology is relatively perfect,the life of the elderly has been extended,the pension problem caused widespread concern in society.The elderly live at home is a main way of pension in the past in China.It is worth pondering that how to make the oldest old to adapt the life of the nursing home.Influenced by traditional ideas,the oldest old don't adapt in nursing home.Their quality of life and subjective happiness sense decreased.This study found that H grandma don't adapt to life the nursing home with the observation and interviews method from the perspective of social work.Inadaptability aspects include the physiological unsuitability,the psychological unsuitability and interpersonal relationship unsuitability.The main reason include herself and our environrment.The study found that it is necessary to integrate social resources to solve the problem because of the diverse needs of the case.So this research use the case management model to intervene the inadaptability problems of the H grandma in the A nursing home according to social support theory and system theory.Researcher ultimately restores H grandmother and the coordinated development of various subsystems,promotes the case adaptation of the life in the nursing home and the integration of advantages resources of H Grandma with more than two months of professional services.The advantages resources includes H grandmother,her family,other old people in nursing home,care workers,volunteers,relatives and original group of H grandmother.Researcher evaluates and summarizes the methods used in the course of practice to meet the service objectives by using the changes of comparative table and the evaluation of others.The evaluation indicate that social worker successfully intervene the inadaptability problems of the case in nursing home.Not only the case adapted the life of the nursing life,but also lighting confidence on life of the case in nursing homes.In addition,this study thinks over four aspects.They includes the case management model,social work intervention process,social work ethics,the role of social work.This study found that the oldest old left the long-term living environment causes the discomfort of the life.It has a great advantage by applying the case management model to the oldest old living the nursing home.We also give some suggestions to the oldest old family and nursing home.This study was conducted in the rehabilitation nursing home.The object of study is transferred from the nursing home staff.This study shows that social workers can effectively promote the old people to adapt the life of the nursing home and make the life more valuable and meaningful in the nursing home.
