The Practice Research Of The Case Management In The Home Care For The Elderly Living Alone

With the deepening of the aging of the population and the core of the family structure in China,the number of elderly people living alone is increasing.Compared with other elderly groups,the elderly living alone are in a more vulnerable position in the whole elderly group due to the dual attributes of solitary living and old age,which need us to give them more care.Elderly people living alone has problems such as high security risks,limited social networks,and lack of mental support.Their needs are also diverse and personalized.Currently,the government and all areas of society are actively seeking solutions for the elderly living alone about the problem of elderly care.The home-based care model has the characteristics of both family care and institutional care,and has been vigorously promoted because of its emphasis on home and multiple services.In recent years,China has vigorously promoted the elderly care industry,improved service levels,and improved the mechanism of elderly care service,laying a solid foundation for home care services for elderly people living alone.This article uses the H community in Suzhou as an example.During a fieldwork to the elderly living alone in the community,it was found that the current home care services for elderly living alone in the community are facing many problems such as the lack of sufficient resources for the elderly in the community,the multiple needs of the elderly living alone cannot be met,and the needs and resources cannot be effectively connected.Traditional community work and group work methods cannot be solved these questions simultaneously.By contrast,the case management mainly provides services to clients by linking and integrating resources,emphasizing the provision of personalized services based on the clients' multiple needs,focusing on cost control,and being suitable for use in the field of home care for elderly people living alone who face multiple difficulties and multiple needs.This article is mainly divided into five parts:the first part is the background and the meaning of research,and research of related literature;the second part is the concept definition,theoretical basis,research ideas and research methods related to the research;the third part is about the situation of home care for the elderly living alone in the H community,this part will be explained from different perspective included the existing resources in the H community,the status of home care for the elderly living alone in the H community,and the needs of home care;the fourth part is the analysis of a practical process which involves a detailed analysis of the case management model to intervene in the home care for the elderly living alone case to reflect the specific operating procedures of case management;the fifth part is to summarize the advantages and areas of case management which could be applied to home care services for elderly people living alone.
