Objective:(1)To understand the main influencing factors of students absent from school due to illness in Wuxi New District,and to master the epidemiological characteristics of students' infectious diseases;(2)To assess the timeliness and effectiveness of the report on the absence of the students;(3)To carry out early Retrospectively collecting and analyzing infectious disease information in students from September 1,2013 to August 30,2014 in "China Disease Prevention and Control Information System",to explore the focus of the region on the prevention and control of infectious diseases;warning analysis of chickenpox conglomeration epidemic situation,evaluate the effect of the absence monitoring system of students on the prevention and control of infectious diseases in schools,Therefore can realize further strengthening the monitoring of students absence due to illness,Improving early warning of infectious diseases,and providing the basis of scientific and effective prevention and control measures in school.Methods:(1)Retrospectively collecting and analyzing monitoring information on Student absence due to illness In 27 schools in Wuxi New District during September 1,2013 to August 30,2014;(2)Retrospectively collecting and analyzing infectious disease information in students from September 1,2013 to August 30,2014 in"China Disease Prevention and Control Information System",to explore the focus of the region on the prevention and control of infectious diseases;Comparing the data of students with infectious diseases in two reporting systems to Analyze the timeliness and effectiveness of students' absence report due to illness;(3)Using EARS-3C model to carry out a retrospective early warning analysis of the information about the absence of school due to chickenpox in a school,and to explore the early warning capability of absence information.The sensitivity,specificity and positive predictive value were used to evaluate the effectiveness of early warning,and the timeliness of early warning was evaluated by using the early warning detection time rate.Results:(1)During 2013-2014 school-year,the absent rate of sick students was 14.68 per 10,000 in Wuxi New District,and the average number of days per student absence was 2.00 days;(2)The main causes of school absences are common cold,varicella,accidental injury,gastrointestinal diseases,hand,foot and mouth disease and bronchitis pneumonia;(3)Sickness absent rate of primary school students is higher than other types of schools;The absent rate of sickness in the lower grades is higher than that of the higher grades.There was no significant difference in the prevalence of illnesses between males and females,but five diseases were higher in males than females includes general cold,chickenpox,accidental injury,gastrointestinal diseases and other disease,rubella and scarlet fever were the opposite;students absence due to illness are seasonal changes,the winter reached the peak,the general cold and chickenpox led to the overall trend;Different street students showed different characteristics of the lack of schooling due to illness,High population density,mobility of the region due to high incidence of infectious diseases;(4)The main diseases of students' infectious diseases are chicken pox and hand,foot and mouth disease;The incidence of the population concentrated in the age group of 5-12 years old;The peak incidence of infectious diseases in children in December and from May to July,similar with the epidemiological characteristics of chicken pox and hand,foot and mouth disease;(5)The report information of students' infectious diseases in the two systems is basically consistent among the three distributions;Chicken pox gathered in the primary school,reported by the school health teacher,concentrated in October to January the following year and from April to June,breakthrough cases accounted for 16.67%;(6)Comparing varicella reported cases of two systems,the proportion of patients with varicella-contagious diseases reported in the absence monitoring system was 66.15%,10.77%without records due to winter and summer vacation,21.53%was missing during the school.There were 202 varicella reports in the absence monitoring system which were not recorded in the infectious disease surveillance system,70.79%of the cases had the medical records.There was no significant difference in the time of reporting the varicella pox between two systems;(7)In the early warning evaluation of the outbreak,absence information showed better sensitivity and specificity with the parallel mode early warning,but the positive predictive value and the detection time rate were lower.Conclusions:(1)Student absence rate due to disease is at a low level in Wuxi New District.It is affected by major diseases,student sex,learning stage,the district,seasonal changes and other factors.Primary school students are the main population of infectious diseases,and chicken pox is the main disease of infectious disease prevention and control;(2)In the case of the discovery and reporting of student infectious disease cases,both the infectious disease reporting system and the absence monitoring system have limited ability to capture students 'infectious diseases,but the infectious disease reporting system has advantages in discovering noncommunicable cases of school students'.The absence monitoring system has a high sensitivity in capturing atypical varicella cases and early detection of suspected cases.In the case of varicella epidemic,there is no warning to the infectious disease reporting system,while the absence monitoring system information can take appropriate warning model found that the epidemic,but in the early warning did not reflect the advantages,but it has advantages in discovering small epidemics from class of grade unit.In order to better achieve early school infectious disease early warning early detection.The two systems should complement each other to strengthen mutual inspection,reduce omission,and improve the report quality;(3)Wuxi New District should strengthen the comprehensive supervision of school health:establishing and improving infectious disease prevention and control system in schools based on absence monitoring;strengthening health education and intervention about the general cold,chicken pox and other major diseases in winter;Strengthening the health care resources and skills of students;taking comprehensive measures to improve the physical quality of students,especially the low-grade students;strengthen medical resources investment and construction in intensive population-intensive areas;increasing varicella vaccine policy support,reflecting the social benefits of public health;strengthening supervision on school infectious disease reporting work to,improve reporting quality.
