A Study On The Social Work Service Of The Elderly Patients With Chronic Disease And Intelligent Home Endowment

In recent years,with the deepening of the population aging and the spread of chronic disease,the number of elderly patients with chronic diseases is increasing,and their pension problems are becoming increasingly serious in our country.How to meet the needs of elderly patients with chronic diseases,improve the quality of their life in old age,become a hot topic of social concern.Based on the development of network technology,the "wisdom of home care" services can provide a more comfortable,convenient,secure and innovative services for the elderly patients with chronic diseases.However,due to the technical,service level,the degree of specialization and other aspects of the restrictions,the wisdom home pension development is also facing many difficulties.Under the guidance of the cognitive theory,demand theory and social support theory,this paper explores the social work service for the elderly with chronic diseases by means of the three basic working methods of case,group and community from the perspective of social work.First of all,this paper researched the present situation and dilemma of the wisdom home care of the elderly with chronic diseases in China by studying the literature,and analyzed the necessity and feasibility of social work intervention.Then,through the field investigation,the paper summarized the needs of the elderly with chronic diseases who lived in the K community,and the present situation of the wisdom home care in the community.Found that,the main problem that the community facing is that the low utilization rate of intelligent information platform,the lack of propaganda,the low professional level,and the provision of services cannot meet the special needs of elderly patients with chronic diseases.In view of these problems,social workers use the professional work of case work,group work and community work to provide professional services for elderly patients with chronic diseases,to meet the needs of elderly patients with chronic diseases in the process of professional services and intelligent home care services to promote and practice.Finally,it summed the effectiveness of social work and reflected,to explore the social work service that the case of psychological counseling,the group work for health education,community work to provide social support to meet the needs of elderly patients with chronic diseases,and make up for the lack of intelligent home care services.
