With the continuous development of the social economy,the people's living standards are constantly enriching and developing.However,at the same time of rapid development,many social problems have emerged.Among them,one of the most prominent social problems is the development of population aging.In addition,there are social problems such as imperfect social welfare system and uneven distribution of social resources,which affect people's lives to a certain extent.In such a social context,the elderly as a special social group,their survival and development face enormous challenges.As the elderly age,the body functions gradually decline,and various chronic diseases gradually emerge,which will bring many inconveniences and problems to the lives of the elderly.While pursuing social and economic development,we cannot ignore the survival needs of people.The elderly are groups that need social care and love.Their survival and development cannot be ignored.In this social context,with the help of the project resources of the internship period,the author starts from the health education needs of the elderly,uses the group work method,conducts research on the chronic disease health education of the elderly,explores effective service methods,and provides the elderly with Better chronic health education services.This research is based on the “Elderly Chronic Disease Health Management Project” of the internship institution.In the early stage,the questionnaire method,interview method and observation method were used to obtain the opportunity of chronic health education for the elderly,the way to obtain health education knowledge and the understanding of chronic disease health education.In these three aspects,we have a comprehensive understanding of the basic situation of chronic disease health education for the elderly in the community where the internship is located.It is found that the elderly generally seldom participate in community health education activities,and lack the channels and opportunities for receiving health education.After analyzing the data,the author summarizes the three major needs of the community elderly in the health education of chronic diseases,including the needs of physical health,the needs of mental health and the needs of social interaction.Based on different needs,six group activities were carried out,mainly from understanding each other,understanding chronic diseases to moderately learning diets,healthy exercise and appropriate emotional relaxation,and then dealing with self-awareness and parting emotions.In the group activities,follow the professional values of social work and the professional concept of social work,and use professional social work methods to meet the various needs of the team members.Studies have shown that group activities are very effective and can meet the needs of team members in terms of physical health,mental health and social interaction.The team members have a high evaluation of the whole group and have gained a lot of new knowledge,new friends and new participation.habit.However,the group work also has some difficulties and problems in the process of intervention.It is characterized by lack of pertinence of activities,limited ability of group workers,insufficient resource integration,etc.,and needs to improve the social workers' own ability and more targeted satisfaction.Improvements in its needs and integration of multiple resources.The author believes that this study aims at the practice and exploration of chronic disease health education for the elderly in urban communities,sums up the lessons learned in the practice process,and provides new ideas for the group work in realizing the needs of chronic health education for the elderly;for better satisfaction The multi-level needs of the elderly provide a practical reference;it has important practical significance for better realizing the social development goal of “having the old,learning,being happy,and being old”.
