| IntroductionSciatica is not an independent disease, it□s a clinical syndrome caused by multiple factors with pain as its main symptom along the sciatica nerve pathway and distribution area. Radicular sciatica is more frequently seen mostly due to lumbar disc protrusion and lumbar spinal stenosis. Paroxysic or rest pain usually occurs in lumbar and legs at one side. The major symptoms are radiatve, scorching or darting pain at waist, hips, back of thigh, posterolateral part of legs, and the lateral part of feet. Epidemiological studies show that sciatica has the highest incidence among all kinds of neuralgia in our body. This disease will seriously affect the quality of life of the patients if they couldnDt be treated timely.Currently there is no generally accepted and effective drug, nor special effects of treatment at home and abroad with sciatica. However, most of the patients can alleviate the symptoms through the conservative treatments with its low cost, simple operation, and non-invasive, so conservative therapy has became the first ladder of the treatment strategies for radicular sciatica. Traditional Chinese Medicine therapy have an exceptional power in treating radicular sciatica under the academic guidance, such as overall concept, the combination of disease and syndrome differentiation, meridians and points. Among them, the acupuncture therapy has a long history, which is well known by the character of simple operation, remarkable curative effects, low cost, small side effects, etc. My tutor, professor Chen summarizes that acupuncture "Yaoyangguan (DU3), baliao (BL31.BL32,BL33, BL34)" have an obvious effect in treating radicular sciatica on the basis of long-term clinical experience without systematic research. This subject carries out the randomized controlled study, in order to objectively evaluate its clinical efficacy.ObjectiveThe aim of this study was to observe the clinical efficacy and safety of treating radicular sciatica by acupuncture "Yaoyangguan (DU3), baliao (BL31,BL32,BL33, BL34)". Another aim was to clear the features and advantages of this group points to provide reliable evidence for the clinical application.MethodsSixty cases were randomly divided into a test group and a control group,30cases in each group. The patients in the test group were treated by acupuncture "Yaoyangguan (DU3), baliao (BL31,BL32,BL33, BL34)"every other day and retaining needle for 30minutes each time with manipulating for twice at interval; while the control group were treated by oral administration of Glucosamine Indometacin Enteric-coated Tablets after meal,twice a day and one tablet each time. In both two groups, the results observed would be recorded before the treatment, every4days in treatment cycle, and three times in following-up period.The clinical observation gone along by using VAS, lumbar JOA, Oswestry disability index scales.In addition, the adverse events were recorded in the course of treatment in order to determine the therapeutic safety. The data were analyzed by statistics through SPSS17.0.ResultsThe statistical analysis of the test group and control group patients by age, gender, course of disease and the baseline of observed indicators such as VAS.JOA and ODI had no significant difference(P>0.05).Therefore,the two groups were comparable.Self-comparison between after and before treatment according to the VAS、JOA and ODI score:the test group had a highly significant statistical difference (PvAs, PJOA, PODI <0.01); while the control group showed significant statistical difference (PvAS, PJOA <0.01, PODI<0.05).Comparison between two groups:The VAS and ODI scores between two groups were significantly different each observation time point during the treatment period (P<0.01), and the means of the test group less than the control group. JOA score between the two groups, showed no statistically significant (P>0.05).There was a highly significant statistical difference in clinical efficacy between the two groups (cure and effective rate:test group80%,control group53.3%; effective rate:test group100%,control group93.3%)The different scores of VASJOA and ODI from the1st time to the following observation time piont between two groups were totally different,exept the VAS (whose scores were no significant difference from the3rd time point).Compared with the end of treatment,the observed indexes of both groups did not distinctly recur in2weeks,1and3monthes after the treatment(all P>0.05),but the VAS and ODI scores of test group were less than control group(PVAS<0.05, PODI<0.01).No serious adverse reaction was found in the two groups.ConclusionThis clinical study proved that acupuncting group points with Yaoyangguan (DU3), Baliao(BL31-34) in treating RS were effective and safe. And this group points had the features and advantages with less acupoints,faster therapeutic effect,more durable and stable.As a result,it was worthy of clinical application for the group points with Yaoyangguan (DU3), Baliao(BL31-34) in treating RS. |
