Based On Data Mining, Zhang Peijun, Director Of The Treatment Of Pancreatic Cancer Patients With Prognostic Factors

Background:According to the statistics of China Cancer Registration Annual Report 2012,The incidence and mortality of pancreatic cancer accounted for 7th and 6th of malignant tumors,respectively,in 2009.Approximately 60%of patients diagnosed with pancreatic cancer have distant metastasis,25%patients with locally advanced,lose the chance of radical surgery.The median overall survival is only 6?9 months;only 15%of the patients were able to perform surgery,the median overall survival was about 15 months,5 years survival rate is about 5%.In recent years,Integrated Traditional Chinese and Western medicine treatment has shown an advantage in improving the symptoms of pancreatic cancer patients,improving the quality of life,increasing efficacy and attenuating toxicity with radiotherapy and chemotherapy,and prolonging overall survival.Objection:Through the analysis of patients with pancreatic cancer in director Zhang Peitong s pancretic thematic clinic,calculate the median overall survival and survival rate of 1.2.3year,and analyze the relationship between the factors and the prognosis of pancreatic cancer,make use of data mining platform,to summarize the clinical experience of Professor Zhang Peitong in the treatment of pancreatic cancer.Method:Part ?:In this study,147 patients were followed up from January 2013 to September 2016 through Chief Physician Zhang Peitong s pancretic thematic clinic,all patients were clinically diagnosed as pancreatic cancer,all patients recieved treatment combining traditional Chinese and Western medicine.recording basic information,tumor,previous treatment through Clinical Registration Form of pancreatic cancer(attached to Appendix).Follow-up by telephone or outpatient every 3 months.Calculate the median overall survival and survival rate of 1.2.3 year,and analyze the relationship between the factors and the prognosis of pancreatic cancer.In addition,analyse overall survival of chemotherapy after radical surgery,chemotherapy in advanced stage.Part ?:Based on scale-free network analysis,utilize Beijing old and famous Chinese medicine experience data warehouse and analysis mining platform to sum up the dialectical law,the method of treatment,the core prescription,the medication rules of Professor Zhang Peitong.Results:Part ?:Up until December 31,2016,a total of 140 cases were followed up.The follow-up rate was 95.2%,of which 29 cases were still alive,the longest has survived 82.9 months,followed up for 1.5 to 82.2 months,the median overall survival was 10.8m,the average overall survival was 15.056�1.078 m,1,2,3 year survival rates was 43%,22%,13%,respectively.Log-Rank test showed that the pathological conditions,distant metastasis,liver metastasis,TNM staging,radical resection,chemotherapy,are prognostic factors for pancreatic cancer(P =<0.01,0.004,0.023,0.001<0.01,0.011),while gender,age,tumor location,lymph node metastasis,radiotherapy showed no significant difference in survival(P?0.375,0.809,0.652,0.309,0.680).The COX regression analysis showed that radical resectionis independent factors affecting the prognosis of pancreatic cancer,among them,radical resection is protective factors for the prognosis of pancreatic cancer.In view of the fact that,in this study,there are more patients accept radical mastectomy and chemotherapy for advanced non radical surgery chemotherapy(44.2%),so analyse these two groups individually,other treatment groups have few patients,not analysis.A total of 32 patients were treated with chemotherapy after radical surgery.The median survival was 19.917m,and the average survival was 24.784�3.308m.Patients were treated with chemotherapy in late stage,the median overall survival was 9.167m,and the average overall survival was 10.653�1.021m.Part ?:Analysing the data mining results,frequency of drug use in the top 22 are vinegar gold rhizome,astragalu,fried horseradish,vinegar curcuma,red peony,poria,vinegar triangular,peony skin,Chinese sage herb,angelica,buttercup toot,bran fry atractylodes,fritillaria thunbergii,capillaris,hedyotis diffusa,vinegar schisandra,mountain sagittaria,scutellaria,anemarrhena,coptis.Drug category of high frequency are antipyretic,analgesic,Jianpi,Xingqi,activating blood circulation,detoxifying drugs,drug resistance on digestion;Drug flavor preffered sweet,bitter,acrid,warm,peaceful,cold,slightly cold,slightly warm;Drug tropism perferred more to the stomach,liver,spleen and heart.Medicine on high frequency are vinegar gold rhizome-fried hawthorn,astragalus-vinegar gold rhizome,astragalus-fried hawthorn,astragalus-red peony,peony skin-red peony,fried hawthorn-red peony,Chinses Angelica-vinegar gold rhizome,peony skin-vinegar gold rhizome,vinegar triangular-vinegar curcuma,vinegar curcuma-vinegar gold rhizome,vinegar gold rhizome-scutellaria,astragalus-vinegar curcuma,Chinese sage herb-buttercup toot,astragalus-poria,vinegar gold rhizome-vinegar triangular.Medicine on high frequency and close degree are vinegar gold rhizome-fried hawthorn,vinegar gold rhizome-astragalus,Chinese sage herb-buttercup toot,vinegar triangular-vinegar curcuma,peony skin-red peony,mountain arrowheads-hedyotis diffusa,fried hawthorn-astragalus,peach kernel-safflower,capillaris-schisandra,fritillaria thunbergii-anemarrhena,auckandia root-amomum,poria-bran fried atractylodes,vinegar rhizoma cyperi-radix linderae,vinegar rhizoma cyperi-rhizoma corydalis,scutellaria,-coptis.The core prescription are Gexia Zhuyu Decoction and Liujun Ermu Decoction.Conclusion:Pancreatic cancer with different situation of tumor,different treatment,they got different prognosis.Combination of TCM and Western medicine can prolong the overall survival of patients to a certain degree.Professor Zhang Peitong advocates the priciple of"wet,hot,poison,sputum,blood stasis,virtual",forming a main idea of "detoxification,heat dampness,blood stasis,phlegm sanjie,care of the spleen and stomach,righteousness,".
