ObjectivePulmonary fibrosis is a kind of interstitial lung disease with unknown etiology and complicated pathogenesis,and so far,there is no specific and effective treatment.Base on the relationship between lung and spleen in TCM theory of "the spleen is the messenger of the lung,and spleen and lung are the relationship between mother and son,so by nourishing the spleen and stomach and then nourishing lung" which is is a common idea in clinical treatment of pulmonary fibrosis,and the research on the pharmacological action and the exact mechanism of this theory is still in the exploration stage.This study intends to establish spleen deficient compound in the rat model of pulmonary fibrosis and sijunzi decoction intervention lung fiber rats,from two angles of spleen deficient and tonifying spleen preliminary discussion from the spleen intervention effects on pulmonary function and pulmonary fibrosis histopathological changes.Through the extracellular matrix,TGF-β1,α-SMA,SP-D such as pulmonary fibrosis related molecular changes to further study on two kinds of interventions function mechanism of the change of lung function,pulmonary fibrosis exploration on the basis of support for the clinical diagnosis and treatment and train of thought.MethodExperimental study on pulmonary function in rats with pulmonary fibrosis that rheum officinale causes spleen deficiency.The spleen deficiency model of rats was established by the combination of rheum officinale and the other factors,which lasted for 30 days.After stopping intragastric administration for 2 days,a group of rats returned to normal feeding(spleen deficiency group);another group of rats were injected with bleomycin(5mg/kg)in the trachea,and then the model of spleen deficiency complicated with pulmonary fibrosis was established(compound group);At the same time,the other group of normal rats were injected in the endotracheal tube with bolomycin,no compound factor,and establish a simple pulmonary fibrosis model(model group).After 28 days,the animals were killed and the experimental materials were taken.During the experiment,the effect of spleen deficiency on pulmonary function in rats with pulmonary fibrosis was evaluated by the animal behavior observation,changes in body weight,lung whole observation,organ coefficient,lung function index,HE staining,Masson staining,Sirius red staining and so on.Experimental study on pulmonary function in rats with pulmonary fibrosis that sijunzi decoction tonifying spleen.SD rats were randomly divided into normal control group,model group,sijunzi decoction low dose group,middle dose group and high dose group.From the 14 day,the rats in the normal control group and model group were given the same volume of normal saline,and the rats in each dose group were given the corresponding medicine at the same time.After 28 days,all the animals were killed and the experimental materials were taken.The overall observation of lung morphology,the quality of weighing organs and the calculation of organ coefficient were finished.The correlated indexes of pulmonary function of rats in each group were detected by lung function detector.HE staining,Masson staining and Sirius red staining were performed in the left lung.Arterial blood gas parameters were measured in rats with abdominal aorta blood,including blood PH(PH),partial pressure of oxygen(PaO2),oxygen saturation(SaO2),carbon dioxide partial pressure(PaCO2).Using immunohistochemi-cal method to observe the expression and tissue distribution of TGF-β1,α-SMA,SP-D protein in lung tissue.ResultsRheum officinale causes spleen deficiency can cause weight loss.Compared with the normal group,lung mass,lung coefficient,FEV0.4/FVC%were all increased in model group(P<0.05),and the indexes of lung dynamic compliance(Cdyn),forced vital capacity(FVC),FVC/lung weight decreased in model group(P<0.05),peak expiratory flow rate(PEF)as well as maximal mid-expiratory flow rate(MMF)decreased in model group(P<0.05).Lung tissue of fibrosis were pathologically characterized by small airway centered interstitial fibros-is,desquamation of epithelial cells and mucus plugs could be seen in the small airway in pathological section of the model group.The indexes of spleen deficiency group were normal after ending intragastric administration of rheum officinale for 4 weeks.Compared with the model group,FEV0.4/FVC%significantly increased(P<0.05),and the area of interstitial fibrosis increased in the compound group’s pathological section.Sijunzi decoction tonifying spleen can significantly decrease the lung coefficient of model group(P<0.05),and increase the weight of pulmonary fibrosis rats(P<0.05),which helps to improve lung function in the whole state.Compared to the model group,each dose group significantly improved the Ri,Re,and Cdyn(P<0.05),increase the FVC,and decrease the FEV0.4/FVC(P<0.05).There were no significant differences among the groups in the detection of arterial blood gas.In Masson staining and picrosirius red staining of collagen,we can find that in every dose of Chinese medicine group collagen distribution are different,but the degree is less than the model group.The results showed that sijunzi decoction could reduce the expression of TGF-β1,α-SMA and improve SP-D protein in lung tissue of rats,reduce the deposition and distribution of collagen,and improve the lung function of rats with pulmonary fibrosis.Conclusion1.Spleen deficiency combined with bleomycin is a potential negative factor in aggravating restrictive lung function of pulmonary fibrosis rats.2.Sijunzi decoction tonifying spleen can improve the pulmonary function of rats with pulmonary fibrosis,and increase body weight of rats.It shows the pharmacological action of Chinese medicine.3.Sijunzi decoction for the improvement of pulmonary function,may be related to reduce the lung tissue consolidation area TGF-β1,α-SMA reduced protein expression,and promote the areas of non-consolidation SP-D protein secretion,ease the process of pulmonary fibrosis.
