Study On Syndrome Characteristics And Quality Of Life Of Patients With Type 2 Diabetes Mellitus With Yin And Yang

Objects:According to a report from International Diabetes Federations,the population of Diabetes Patients has reached the number of 41,500,000 and predicted to reach 62.2 million in 2040.Diabetes mellitus,as one of endocrine and metabolic diseases,will become a common and urgent problem in the future.The occurrence and development of DM are related with many factors such as living habits,diet and rapid aging.DM is considered as"dispersion-thirst disease" based on TCM theory.And based on the constitutional theory of TCM,it can be concluded that throughout the process of human life,a relatively socially and naturally adapted characteristic of human body/bodies is formed,which is based on the fact that humans are born with natural and acquired characteristics of physiological form and function,mental structure and inherent hidden traits.The occurrence of DM is fundamentally related to the differentiation of human body’s wellness and constitution.Based on TCM theory and the constitutional classification of three yin and three yang put forward by Professor Zhao Jin-xi,this research of 2DM patients constitutions is to further explore the correlations of generalities and peculiarities,the clinical features,quality of life and the effect of anxiety-depression between Tai Yin and Yang Ming constitutions.Method:A research of was conducted among 2DM inpatients and outpatients with Tai Yin or Yang Ming constitution in Dongzhimen Hospital from 2016.01 to 2017.01.This research described and analyzed the data of patients’ demographics,anthropometric parameters and lab results by conducting several questionnaires including "constitution of Three yin and Three yang","Diabetes Specific Quality of Life Scale","Self-rating depression scale" and "The 8-item Morisky Medication Adherence Scale".SPSS 20.0 was used for data analysis.For the questionaries,patients’ personal information,symptoms,tongue and pulse were recorded after inquiring patients in person.By exploring the correlations of patients’ constitutions,syndromes,quality of life,anxiety-depression state,mental state,glucose level,compliance and complications,and analyzing their generalities,peculiarities and the reasons behind,to recommend TCM preventions and interventions for 2DM patients and improve their quality of life.Results:1.There is no statistic significant correlation of patients’ age,jobs,education,diet and exercise habits,complications,quality of life scales and depression scales found between Tai yin and Yang Ming constitutions of 2DM patients.2.Patients with both smoking and drinking habits from both groups show statistic significant differences,19.3%for Tai yin constitution is lower than Yang Ming constitution that is 36.2%.3.In terms of anthropometric parameters,the differences between the two groups show statistical significance.For data from Tai yin constitution group,normality is shown(p=0.111),with an average of 25.76±3.90.For Yang Ming group,data doesn’t show normality(P=0.014),with an average of 26.59±4.43.4.Difference of living habit shows statistic significance between the two groups(P=0.091),patients from Tai yin group sleep early while patients from Yang Ming group wake up early.5.Syndrome-wise,the difference between the two groups shows statistic significance(P=0.004),for Tai yin group most patients show phlegmatic hygrosis syndrome while most patients from Yang Ming group show heat accumulation syndrome.6.In terms of medicine application compliance,data shows statistic significant difference(P=0.031)between the two groups,Tai yin group(9.7%)is lower than Yang Ming group(23.4%).7.Patients’ anxiety scales are significantly different between the two groups(p=0.02),Tai yin group with the number of 32.3%is higher than Yang ming group with a number of 10.6%.Conclusions:Patients with Yang Ming constitution more often have smoking and drinking habit than patients with Tai yin constitution.Patients with Yang Ming constitution wake up early while patients with Tai yin constitution sleep early.Patients with either Yang Ming or Tai yin constitution are overweighed according to their BMI and prefer greasy food in terms of eating habit.Syndrome-wise,phlegmatic hygrosis syndrome is most seen among patients with Tai yin constitution while it’s heat accumulation syndrome for patients with Yang Ming constitution,an both groups of patients show secondary sthenic syndrome.In terms of complications,diabetic retinopathy is the most common.Patients with Yang Ming constitution have more hypotension and abnormal blood lipid histories and the ratio of patients’ family history of DM is 2.4 times higher than Tai yin constitution group.Also patients with Yang Ming constitution show lower compliance of medicine applications.
