| Objective:The aim of this essay was to evaluate the competency of emergency physicians from county-level general hospitals, taking those from Beijing hospitals as research object.By combination with previous research achievements and construction of competency model for emergency physicians from county-level general hospitals, this essay was designed to provide reference and suggestions in terms of training, performance review and continue education for county-level emergency physicians.Methods:(1)Using literature research method, the literature and information related to domestic and international emergency physician competency studies were summarized.Through frequency analysis,an initial secondary index library specific to the competency of emergency physicians was established.(2)Using expert consultation method,a questionnaire on the competency of emergency physicians from county-level general hospital was built, including two-round survey(7 experts in the first round,13 experts in the second). With constant revision and improvement, the final version questionnaire on the competency of emergency physicians from county-level general hospitals was prepared.(3)Using questionnaire method,an online survey on all physicians in the emergency departments of 15 county-level general hospitals in Beijing was conducted.(4)Using mathematical statistics method, the survey results were thoughtfully analyzed, including calculation of the reliability and validity of this questionnaire, collection of basic hospital information with the survey on emergency departments directors of 15 hospitals, gather of the respondents profile from the first part of questionnaire(including gender, age, education e.tc.),analysis of the second part by exploratory factor analysis, extraction of a competency index system for emergency physicians from county-level general hospitals.Excel 2013 was used to record data.SPSS 19.0 was used for analysis.Results:(1)This research compared the competency models of emergency physicians collaboratively established by the Accreditation Council for Graduate Medical Education, the European Society for Emergency Medicine,Chen Xiaoyun research team from China-Japan Friendship Hospital,Sun Dongmin research team from China Meitan General Hospital as well as Qu Lijuan research team from the Fourth Military Medical University, and took the results of comparative analysis as the initial secondary index library specific to the competency of emergency physicians.(2)Through expert consultation,a questionnaire on the competency of emergency physicians from county-level general hospitals was established. The questionnaire included two parts, with profile collection in the first part, and 39 competency-related questions in the second.(3)Based on the above,a questionnaire survey was conducted and answers were collected and analyzed. Results showed that 268 questionnaires were returned, with 260 qualified. The effective rate was 97%,showing a high level of response. The credibility value was 0.961,the validity value was 0.938,which indicated high credibility of this questionnaire and the data was usable. The results of Bartlett spherical test showed P<0.05,meaning the results had statistical significance and 39 indexes were highly correlated and suitable for factor analysis. The factor analysis result was thus credible. Conclusion:In this research,260 valid questionnaires were returned, about seven times the overall questions. The research data was sufficient.According to study, the competency model of emergency physicians from county-level hospitals was composed of six core competency indexes and corresponding secondary indexes. There were 6 primary indexes, including management ability, emergency service, intrinsic quality, communication ability, emergency operation skill and emergency knowledge.Secondary indexes include 11 of management ability,7 of emergency service,5 of intrinsic quality,6 of communication ability,5 of emergency operation skill and 5 of emergency knowledge.Based on the competency index system of county-level hospitals, this essay established a "petal model" of competency for emergency physicians from county-level hospitals. It may provide theoretical basis for researches on the competency of county-level emergency physicians, and also provide reference for grassroots level health human resource management as well as physician training. |
