Distribution Of TCM Syndromes Of Varicocele And Its Effect On Testicular Function

Objective:To investigate the distribution of common TCM syndromes in Varicocele(VC)and its correlation with testicular function.Methods:A questionnaire was developed.216 cases of VC patients were collected from the department of Andrology of China-Japan Friendship Hospital and their TCM dialectical classification were performed.The result of testicular volume,the maximum diameter of bilateral varicose veins,semen quality,and sexual hormone were collected and the database was established.SPSS 20.0 was used for statistical analysis.Results:The TCM syndromes of VC were followed by 34 cases of damp-heat accumulation syndrome(15.57%),32 cases of bleeding stasis syndrome(14.81%),29 cases of kidney-yang deficiency(13.43%),22 cases of kidney-yang deficiency and cold liver coagulation syndrome(10.19%),20 cases of liver-qi stagnation syndrome(9.26%),17 cases of spleen deficiency syndrome(7.87%),14 cases of cold liver coagulation syndrome(6.48%),12 cases of kidney deficiency syndrome(5.56%),11 cases of liver-yin and kidney-yin deficiency(5.09%),9 cases of kidney-yang deficiency and liver-qi stagnation syndrome(4.17%),7 cases of unidentifiable were(3.24%),3 cases of liver-qi stagnation and cold liver coagulation syndrome(1.39%),3 cases spleen-qi deficiency and liver-qi stagnation syndrome(1.39%),1 case of damp heat retention and liver-qi stagnation syndrome(0.46%),1 case of spleen-qi deficiency and cold liver coagulation syndrome(0.46%),and 1 case of spleen-qi and kidney-qi deficiency syndrome(0.46%).There were no significant difference in body mass index(BMI),semen PH,and the levels of estrogen(E2),testosterone(T),and follicle stimulating hormone(FSH)between different syndromes.The bilateral testicular volume and luteinizing hormone(LH)level in patients with kidney-yang deficiency and liver-qi stagnation syndrome were significantly higher than those in other syndromes.The maximum diameter of bilateral varicose veins in patients with kidney-yang deficiency and cold liver coagulation syndrome were significantly lager than those in other syndromes.The volume of semen in patients with kidney deficiency syndrome were significantly less than those in other syndromes.The viability and forward movement sperm rate in patients with kidney-yang deficiency were significantly lower than those in other syndromes.The percentage of normal sperm morphology in patients with blood stasis syndrome significantly lower than those in other syndromes.Conclusion:The main syndrome distribution of varicocele in TCM syndromes is followed by damp heat accumulation syndrome,blood stasis arrest syndrome,kidney-yang deficiency syndrome,kidney-yang deficiency and cold liver coagulation syndrome,liver-qi stagnation syndrome,spleen-qi deficiency syndrome,cold liver coagulation syndrome,kidney-qi deficiency syndrome,liver-kidney yin deficiency syndrome,kidney-yang deficiency,and liver-qi stagnation syndrome.There are significant differences in testicular volume,maximum diameter of varicose veins,semen quality,and hormone levels among different syndromes of varicocele,which provide a basis for clinical diagnosis and treatment.
