Research On The Rule Of Drug Use Of Diabetic Peripheral Neuropathy Based On Literature

Diabetes mellitus is one of the chronic diseases that seriously affect human health at present.As one of the chronic complications of diabetes,it's disability rate is high.It is a symptom of peripheral nerve dysfunction,except other nerve injury factors.The clinical manifestations include limb numbness,pain,cold and other sensory abnormalities.Clinic symptoms include tendon reflex,position and vibration sense weakened or disappeared.Western medicine thinks that its pathogenesis mainly includes the theory of oxidative stress,abnormality of non glycosylation,hyperactivity of polyol pathway and activation of PKC pathway,but there is no consensus on the same system at present.About treatment,western medicine mainly in the improvement of blood glucose,blood lipid metabolism,while paying attention to antioxidant stress,improvement of circulation,nutritional nerve and other aspects.DPN belongs to the diabetes rheumatism authentication scheme of traditional chinese medicine(TCM),the treatment is based on the syndrome differentiation,internal and external washing methods,but not many comprehensive research,so through the traditional Chinese medicine literature search,to excavate doctors in internal and external washing of DPN treatment rules and precautions,for clinical treatment of diabetic peripheral neuropathy to provide ideas and reference.Objective:By retrieving the relevant literatures of diabetic peripheral neuropathy which conform to the inclusion criteria,the rule of selecting prescription and precautions for treating diabetic peripheral neuropathy by Chinese herbal medicine and foot bath were summarized by collation and data mining.Methods:Based on the literature data of oral diabetes and peripheral neuropathy treatment in recent 10 years,we searched the related articles in CNKI from January 1,2007 to 31,October 2017 in Chinese journal full text database of CNKI.The literature after the screening,will be involved in the literature of traditional Chinese medicine prescriptions,and other related data,using Excel,spass and other related software frequency,correlation statistics,and to explore the internal and external treatment of TCM syndrome type washing syndrome of diabetic peripheral neuropathy,prescription and precautions.Result:We retrieved 1986 articles about the treatment of oral TCM and 436 artcles were brought into statistics in these papers involving 24 syndromes?12 syndrome elements?116 prescriptions and 159 TCMs.Six types of syndromes were appeared more than 50 times,which were obstruction of collaterals by blood stasis?qi deficiency and blood stasis?yang deficiency and cold coagulation?deficiency of both qi and yin?blood deficiency and blood stasis and yin deficiency with blood stasis,respectively.Five types of syndrome elements were appeared more than 100 times,which were blood stasis?qi deficiency?yang deficiency?yin deficiency and cold coagulation,respectively.Seven types of prescriptions were appeared more than 100 times,which were yang supplementing five returning decoction?taohong siwu decoction?decoction for Removing Blood Stasis?dangguisini Decoction?decoction of astragali and cinnamon?decoction of liu wei di huang and shemnai decoction,respectively.Fourteen types of TCMs were appeared more than 100 times,which were astragalu?angelica sinensis?chuanxiong?cassia twig?shudi?safflower?suberect spatholobus stem?peach keme?lumbricus?white peony root?salvia?achyranthes root?radix rehmanniae and red peony root,respectively.The main prescriptions were medicine for activating blood circulation to dissipate blood stasis?medicine for enriching blood?medicine for invigorating qi?medicine forinvigorating yin,which were collocating with medicine for Invigorating qi and warming through blood circulation?medicine for nourishing yin,nourishing blood and activating blood and so on.We retrieved 370 articles about wash-outside TCM treating DPN and 172 artcles were brought into statistics involving 139 TCMs in these papers.Eighteen types of TCMs were appeared more than 30 times,which were safflower?cassia twig?chuanxiong?suberect spatholobus stem?angelica sinensis?tuberculate speranskia herb?astragalu?achyranthes root?salvia?red peony root?lignum sappa?olibanum?peach kerne?myrrh?Chinese Mugwort Leaf?Lycopodii herb?lumbricus and asarum,respectively.The main prescriptions were medicine for activating blood circulation to dissipate blood stasis?medicine for dispelling cold?medicine for nourishing blood and medicine for invigorating qi.Out-side TCM treatments show more flexibility than oral TCM treatment and in the relationship of medicines,drug pairs that presents more than 50 supports were safflower and cassia twig?cassia twig and chuanxiong?safflower and chuanxiong?safflower and astragalus,respectively.The aeverage time is 30 mins for outside washing and the best water temperature were varied from 38 ? to 41 ?.Conclusion:1.Diabetes rheumatism authentication scheme starts with deficiency of both qi and yin and ends with deficiency of both yin and yang,simutaneously in company with obstruction of collaterals by blood stasis.obstruction of collaterals by blood stasis?qi deficiency and blood stasis and yang deficiency and cold coagulation are common clinicals syndromes.2.In oral TCM treatment,yang supplementing five returning decoction?taohong siwu decoction?decoction for Removing Blood Stasis?dangguisini Decoction?decoction of astragali and cinnamon are commonly used supplying with medicines for activating blood circulation to dissipate blood stasis?enriching blood and nourishing qi and yin.3.In outside-washing TCM treatment,medicines for activating blood circulation to dissipate blood stasis medicine for dispelling cold?medicine for nourishing blood and medicine for invigorating qi are commonly applied for treatments with more flexible amount in use.4.There presents consistence between oral TCM treatment and outside-washing TCM treatment are unified.To improve curative effects and reduce risk,oral TCM treament can combined with outside-washing TCM treatment on the premise of reasonable control of water temperature and outside-washing time.
