A Study Of The Problems And Their Countermeasures To Implement The Long-term Care Insurance System In China

China’s population is aging rapidly and deeply,which is the only country in the world with a population of over 100 million.The size of the large elderly population and the growing and aging population have led to a rapid increase in the number of disabled people.In the past,the responsibility of taking care of the old man shall be borne by the family,family play the important part in endowment,but as the economic system reform,population flow,people values change and family size miniaturization,core structure,family endowment function gradually weakened and the social security system is imperfect,long-term care costs high,it is very difficult for the old man to satisfy the basic demand,some old people even face the survival crisis.In this context,a long-term care insurance system in line with China’s national conditions as soon as possible becomes an important means to resolve the risk of disability.Building long-term care insurance system can not only improve the social security system in our country,more important is to provide diversified,sustainable and high quality nursing service for disability,and improve the quality of disability,ensure its to live more dignified.This thesis discusses the basic concept of long-term care insurance system,disabled elders,and pension service,and discusses the practical connotation of nursing,long-term care,long-term care insurance and long-term care insurance system,clarify the relationship between the endowment services and long-term care services;On this basis,selecting Qingdao,Nantong and Changchun three long-term care insurance pilot cities,by introducing the pilot cities pilot basic situation of long-term care insurance system,combined with the existing problems in the process of pilot,analyzes our country possible obstacles and problems for establishing long-term care insurance system;Next,in using the experience of developed countries and domestic pilot cities,on the basis of the basic framework of the long-term care insurance system in China is discussed,and puts forward some basic ideas,from the fund raising,fund pays,nursing service content and provide subject and relevant supporting system,etc;and according to China’s national conditions,puts forward the measures to establish a long-term care insurance system.The main ideas and innovations of this thesis is:(1)in order to make it easier for long-term care insurance system,at the same time in order to ensure the independence of the insurance and the stability of funds,long-term care insurance system in our country can adopt "government + company + individual + medical insurance fund transfer" the four corners of the payment pattern,which requires the use of actuarial methods to reasonably determine the government,enterprise and individual capture to expend scale and proportion of medical insurance fund transfer.(2)the construction of China’s long-term care insurance system should follow the principle of "home-based,community-based,and institutional supplement".This principle applies not only to towns but also to rural areas;In rural areas,long-term care insurance system should be implemented to solve the problems of nursing service.It should be the top priority to build a rural community platform and accelerate the construction of rural communities.(3)from the welfare pluralism and quasi public product theory perspective,China’s supply of nursing service can no longer rely on family or government,the elderly care problem is the responsibility of the whole society,the government should through the establishment of incentive system to encourage the social main body to actively participate in,to build an all-round social support network system which contains the family and the government,enterprises,non-profit organizations and volunteers and so on.(4)Stressing the importance of the nursing screening system,nursing personnel training system and related facilities,such as performance evaluation and quality supervision system,if these supporting system is not perfect,the long-term care insurance system will be an empty shell.
