Chronic wound is a kind of common clinical complication,which is usually caused by burn,postoperative wounds,trauma,pressure ulcers,diabetes,atherosclerosis and other primary diseases.Wound healing is a complex physiological process,there are many factors involved in the regulation of wound repair.Since the cause of the wound is complex and diverse,easy to relapse,hard to cure completely and rarely there may be cancerous,which made a serious threat to the patients' health.In addition,due to the production of wound usually destroyed dermal layers,to some degree,the pain of the wound can affect the patients' quality of life seriously.Thus,pain control can not be ignored in the process of wound healing.Traditional debridement,high oxygen and ultraviolet radiation technology can relieve the wound infection,but they can't heal the wound completely.The best results can be obtained by combined them with the wound healing medicines.With the development of traditional Chinese medicine and in-depth study of wound healing mechanism,a variety of Chinese medicine extracts in wound healing has came into our eyes.Aloe vera is a kind of traditional Chinese medicine which has been used as a medicine for the treatment of wound healing for a long time.At present,the source of the efficient ingredient reported was various.The clinical application of aloe vera preparations are mostly prepared according to traditional experience and lack of sufficient theoretical support.In this article aloe vera gel powder is chosen as model drug because its source is rich and the production cost is low,so it is more suitable for industrial production.We study the mechanism of aloe vera gel powder at the molecular level in order to provide adequate theoretical basis for clinical application.In order to relieve the wound pain,ropivacaine hydrochloride was used to combining with aloe vera.Ropivacaine hydrochloride is a long-acting amide local anesthetic,it has a lower neurotoxic and cardiotoxic within the effective treatment concentration,so it's the ideal analgesic drug for trauma pain.Currently,there isn't compound preparations of aloe vera and ropivacaine hydrochloride on the market.This paper discussed the proliferation and collagen metabolism mechanisms of aloe vera gel powder on human dermal fibroblasts,and developed a compound aloe vera transdermal gel.The preparation in the form of local transdermal drug delivery into the dermal layer of skin,it can promote wound healing and while also can relieve the pain of the wound effectively.Firstly,the primary culture system of human dermal fibroblasts has been established with explant culture technique.The morphology of primary cells was observed by inverted microscope at different periods of time.We collected the third generation of primary cells for routine HE staining and immunohistochemistry staining of vimentin,and the growth curve before and after cryopreservation were drawed.The results showed that the Vimentin expressed as positive,so the primary cells proved to be human dermal fibroblasts and it can be used for future research.This paper taking aloe vera gel powder as model drug to study the proliferation and hydroxyproline synthesis on primary cultured human dermal fibroblasts.Besides that,the mRNA and protein expression of I,? collagen were also researched.The results showed that aloe vera gel powder in the medium after 24 h,48h and 72 h,the lowest concentrations that can promote proliferation of human dermal fibroblasts were 5mg/mL,1.25mg/mL and0.3125mg/mL respectively.After cultured for 72 h,the hydroxyproline contentof aloe vera gel powder from 0.3125mg/mL to 2.5mg/mL had significant differences compared with the blank group(P<0.05,P<0.01).The mRNA expression of collagen I with aloe vera gel powder from 1.25mg/mL to2.5mg/mL showed significant differences compared with the blank group(P<0.01)after cultured for 72 h,and the mRNA expression of collagen? with aloe vera gel powder for 2.5mg/mL has significantly increased(P<0.01).After incubated for 72 h,the expression of I,? collagen with aloe vera gel powder from 2.5mg/mL to 5mg/mL along with significant differences compared with the blank group(P<0.01).The mechanical scratch model of human dermal fibroblasts was established in order to investigate the repair capacity of human dermal fibroblasts between the edge of the scratch and the peripheral monolayer.It turned out to be that the wound healing were in good condition with aloe vera gel powder from 1.25mg/mL to 5mg/mL and the shape of cells was normal.This may suggested that aloe vera gel powder can promote proliferation or migration of human dermal fibroblasts in the scratch areas.In order to study the prescription of compound aloe vera transdermal gel,this paper took drug 24 h cumulative permeability as examining index and used the Box-Behnken design to select out the optimal prescription.The results showed that the 24 h cumulative permeability of aloe vera and ropivacaine hydrochloride can reach 71.27% and 94.87%,the pH is about6.2~6.3.We prepared three batches of compound aloe vera transdermal gel according to the optimal prescription,and then evaluated its quality.The results indicated that the gel looks like pale and translucent at room temperature,the pH is about 6.2~6.3 and has no stratification after high speed centrifugation.To determine the two drugs' content of the compound preparation,uv-visible spectrophotometer method and HPLC assay wereadopted.The precision,stability and recovery of methods were in line with the provisions.The content of aloe vera gel polysaccharide and ropivacaine hydrochloride were between 90% and 110% of labeled amount.Finally,we took SD rats as experimental subjects,the rats were given drug-loaded gel and blank gel once a day for 7 days.The erythema and edema of skin were observed.Furthermore,the skin tissue was collected to make tissue sections and observed it by optical microscope.Histological observation showed that the drug-loaded gel and blank gel had no apparent irritation on rats skin.
