ObjectiveTo realize current situation about exclusive breastfeeding of the early infancy to the second children.Analysis of influencing factors about exclusive breastfeeding of the early infancy and the tendency of exclusive breastfeeding rate.Providing some references for managers and future researchers to make interventions,increase the rate of early exclusive breastfeeding and quality of breastfeeding to the second children.MethodsThe longitudinal research design was used.320 mothers and infants from 3 large hospitals in Zhengzhou from 2017 March to 2017 April were surveyed.They were follow-up surveyed when babies were 42 days,3 months and 6 months.There were two parts:The first part: Maternal and child situation questionnaire(one),the breastfeeding self-efficacy scale-short form(BSES-SF),and a breastfeeding knowledge scale of ZhaoMin were used to survey mothers and infants during hospitalization after childbirth.The second part:Maternal and child situation questionnaire(two),the Edinburgh Postnatal depression scale of Guo Xiujing were used when babies were 42 days,3 months and 6 months.SPSS 21.0 was uase to analyze the data.The meanąstandard deviation,frequency,percentage were used for data statistical description.Chi-square test,Kruskal-Wallis non-parametric test were used to conducted single factor analysis;Logistic regression analysis was used to find out influencing factors.P < 0.05 for the difference was statistically significant.Results1.There were 320 research objects.Four of them withdrewed from the study.There were two fall off.314 pairs of maternal and child participated,completed this study.The rate of lost to follow-up was 1.9%.2.Current situation about exclusive breastfeeding of the early infancy to the second children in Zhengzhou was as follows: Exclusive breastfeeding rate of the second children during hospitalization after giving birth was 55.9%.Exclusive breastfeeding rate of the first children during hospitalization after giving birth was 50.8%.There was no statistically significant difference between exclusive breastfeeding rate(P =0.370).Exclusive breastfeeding rate when the second children 42 days was 53.8%.Exclusive breastfeeding rate when the first children 42 days was 49.2%.There was no statistically significant difference between exclusive breastfeeding rate(P =0.428).Exclusive breastfeeding rate when the second children 3 months was 39.2%.Exclusive breastfeeding rate when the first children 3 months was 46.9%.There was no statistically significant difference between exclusive breastfeeding rate(P =0.179).Exclusive breastfeeding rate when the second children 6 months was 11.3%.Exclusive breastfeeding rate when the first children 6 months was 16.4%.There was no statistically significant difference between exclusive breastfeeding rate(P =0.191).Exclusive breastfeeding rate when the second children 3 months and 6 months were far lower than the target of our country.3.Influencing factors about exclusive breastfeeding of the early infancy to the second children were as follows(The influencing factors in this study were mainly the factors those have obvious influence on the exclusive breastfeeding of the second child,but have no obvious influence on the first child):3.1 Influence factors of exclusive breastfeeding during hospitalization after giving birth were as follows:(1)Postpartum physiology:Blade/incision pain(severe pain OR=7.297,P =0.023);(2)Knowledge:Score of Breastfeeding knowledge questionnaire(OR=1.153,P =0.020),intervals of time between two times of pregnancy(OR=4.627,P < 0.001);(3)Social support:With or without teaching knowledge about pregnancy complication(OR=8.193,P <0.001).3.2 Influence factors of exclusive breastfeeding when babies were 42 days were as follows: The age of the baby's father(thirty and above OR=4.284,P =0.016).3.3 Influence factors of exclusive breastfeeding when babies were 3 months were as follows:(1)Physiological status:Mothers' age(thirty-five and above OR=5.173,P =0.001);(2)Family support:The attitude of other family members to exclusive breastfeeding(nonsupport OR=3.844,P =0.022).3.4 Influence factors of exclusive breastfeeding when babies were 6 months were as follows: Working state of mothers(no return to work OR=0.047,P =0.001).4.The tendency of exclusive breastfeeding rate in early infancy to the second children in Zhengzhou was as follows: Exclusive breastfeeding rate during hospitalization after giving birth was 55.9%.Exclusive breastfeeding rate when the second children 42 days was 53.8%.Exclusive breastfeeding rate when the second children 3 months was 39.2%.Exclusive breastfeeding rate when the second children 6 months was 11.3%.Exclusive breastfeeding rates continued to drop.Conclusion1.Exclusive breastfeeding rate between the first children and the second children during hospitalization after giving birth,when the children 42 days,when the second children 3 months and 6 months were nondistinctive in Zhengzhou.Exclusive breastfeeding rate in the early stage of the second children continued to drop.Exclusive breastfeeding rate when the second children 3 months and 6 months were far lower than the target of our country.Exclusive breastfeeding rate when the second children 6 months was lower than that of the whole infant in China.Exclusive breastfeeding rate when the second children 3 months and 6 months were lower.They need to be improved.2.The influencing factors of different time node for exclusive breastfeeding of the second children were different.There needed to take corresponding measures,in order to increase the rate of exclusive breastfeeding of the second children,extend the time of exclusive breastfeeding.
