Objective:To explore the clinical significance of platelet to lymphocyte ratio(PLR)and red cell distribution width(RDW)in prediction of the severity of preeclampsiaMethods:Totally 203 cases of parturients treated in the obstetrics department of our hospital from January 1,2015 to December 31,2017 were divided into mild preeclampsia group(MPE group,n=96)and severe preeclampsia group(SPE,n=107),in the meanwhile,the normal pregnant women were selected as control group(n=110).The routine blood indexes including Neutrophil(NEU),lymphocyte(LYM),white blood cell(WBC),absolute platelet(PLT),hemoglobin(HB)and RDW of the patients in each group on admission were detected,and the PLR was calculated to comparatively analyze the PLR and RDW of the patients in each group.Results:1.The age,gravidity and abortion times of the pregnant women in the MPE group and SPE group were all higher than those in the normal pregnant group(P<0.05);the gestational age and the newborn birth weight of the pregnant women in the MPE group and SPE group were all less than those in the normal pregnant group,furthermore,the gestational age and the newborn birth weight of the pregnant women were all on a declining trend along with the increase of the severity of preeclampsia(P<0.001).2.The peripheral blood PLR of the pregnant women was positively correlated with the gestational age and the newborn birth weight,and was negatively associated with the systolic pressure and diastolic pressure;there was a positively correlated relationship between the RDW and systolic pressure or diastolic pressure.3.The RDW and LYM of the pregnant women in the MPE group and SPE group were all higher than those in the normal pregnant group,which were all elevated with the increasing of the severity of PE;the PLR and PLT of the pregnant women in the MPE group and SPE group were all lower than those in the normal pregnant group,which were all decreased with the increasing of the severity of PE.The differences of NEU,WBC and HB had no significance in statistics.4.The severity of PE was increased with the elevation of RDW and LYM;the severity of PE was increased with the decrease of PLR and PLT.5.The area under ROC curve of the PLR diagnosis was 0.665,which was statistically significant(P<0.05),suggesting that the PLR had diagnostic significance on PE.The sensitivity,specificity and cut-off value of PLR was 61.6%,67.5%,111.13,respectively;the area under ROC curve of the RDW diagnosis was 0.576,which showed statistical significance(P<0.05),indicating that the RDW had diagnostic significance on PE.The sensitivity,specificity and cut-off value of RDW was 67.0%,48.8%,13.95%,respectively.Conclusion:Both of PLR and RDW are capable of predicting the severity of preeclampsia,and the severity of preeclampsia is increased in the wake of the decrease of PLR or the elevation of RDW.The PLR and RDW can be regarded as the cheap and convenient commonly-used reference indexes for prediction of the severity of preeclampsia. |
