| Research Purpose: Through the combined detection of serum ?-HCG,progesterone and CA125 levels,analyzed the correlation between the detected value and the outcome of pregnancy women with the symptom of early threatened abortion was.Research Object:Seventy-four patients who visited the Affiliated Zhongshan Hospital of Dalian University from September 2015 to October 2017 with the symptom of threatened abortion were selected as the observation group,aged(21-40)years,mean age(30.9±4.6)years,menopause(36-65)days,the average number of menopause days(44.6 ± 5.8).After systematic treatments were given to the patient's,according to the pregnancy results,74 patients with threatened abortion were divided into 2 groups,namely: 1 successful group(pregnant women with with the symptom of threatened miscarriage during early pregnancy,and symptoms relieved or disappeared after systematic treatments)(n = 44),age(21-38)years,mean age(30.8 ± 4.8)years,menopause(37-56)days,mean menopause days(44.7 ± 5.0)days.2 Abortion group(pregnant women with threatened miscarriage during early pregnancy,failure of fetal failure after pregnancy treatment,termination of pregnancy)(n=30 cases),age(24-40)years,average age(31.1±4.3)years,pregnancy(37-65)days,average(44.0±6.9)days.At the same time,the normal pregnant women without symptoms of abortion and signs who were undergoing birth examination in the same period of our hospital were selected as the control group(n=50 cases).Age(22-40)years,mean age(30.3±5.1)years,pregnancy(38-63)days,average(48.2±5.6)days.All pregnant women who were enrolled in the study performed HCG/urine pregnancy test to indicate pregnancy and were diagnosed as having a single intrauterine pregnancy by ultrasound.Research Methods:(1)collected and organized the basic data of each group of patients,name,age,menstrual cycle,menopause days,birth history,and personal medication and check the gestational age.(2)Observing group and control group took blood of 4 ml of elbow venous blood on the day of the visit,and measured serum progesterone,?-HCG,and CA125 concentrations in maternal serum by chemiluminescence analysis.(3)Statistical methods: All data were processed by SPSS 20.0 statistical software,and the measurement data was represented by(x±s),and analysis of variance was applied among the groups.The difference was statistically significant at P<0.05.The successful group and the control group in total 90 patients were followed up to 28 weeks of gestation.The abortion group were followed up to the termination of pregnancy.Patients with symptoms of threatened miscarriage were treated with bed rest and hormone supplements.Research Results: After symptomatic treatment,all groups were followed up to 28 weeks.In the observation group,44 cases were successfully fetuses,aged(21-38)years,mean age(30.8±4.8)years old,menopause(37-56)days,and mean menopause days(44.7±5.0).Thirty cases of abortion were inevitable miscarriage,age(24-40)years,mean age(31.1±4.3)years,pregnancy(37-65)days,average(44.0±6.9)days.1.There was no statistical difference between the observation group and the control group(P>0.05).2.1 The progesterone level in the observation group was 24.1±9.39 ng/ml,and the progesterone level in the control group was 45.6±10.8 ng/ml before the fetal control.The serum progesterone level in the observation group was lower than that in the control group.(P<0.05);2 The serum HCG level before treatment in the observation group was 12758±5044m IU/ml,and the serum HCG level in the control group was30306±11112m IU/ml.The serum HCG level in the observation group was lower than that in the control group(P <0.05);3 The serum CA125 level in the observation group before treatment was 31.6±11.1 IU/ml,and the CA125 level in the control group was14.35±2.63 U/ml.The CA125 level in the observation group was higher than that in the control group(P<0.05)3.1 Serum progesterone level in pre-treatment group was27.0±7.2ng/ml,and serum progesterone level was 19.7±10.5ng/ml before miscarriage in the miscarriage group.Serum progesterone levels were high in the successful fetal-prevention group.In the unavoidable miscarriage group(P<0.05);2 The serum HCG level before successful treatment was 16031±2022m IU/ml,and the serum HCG level in the unavoidable miscarriage group was 7956±4229m IU/ml.The serum HCG level was higher in the successful miscarriage group.Inevitably abortion patient group(P<0.05);3 patients with abortion success group The pre-treatment serum CA125 level was 25.5±5.5 IU/ml.It was difficult to avoid the pretreatment CA125 level in the abortion group was 40.5±11.2 U/ml,and the CA125 level in the successful fetal control group was lower than that in the refractory abortion group(P<0.05);4.HCG There was a significant negative correlation with threatened abortion(rs<0.00,P<0.05),that is,the higher the serum concentration of HCG,the better the prognosis of patients with early threatened abortion;there was a significant negative correlation between progesterone and threatened abortion(rs <0.00,P<0.05),that is,the higher the progesterone serum concentration,the better the prognosis of patients with early threatened abortion;there is a significant positive correlation between CA125 and threatened abortion(rs>0.00,P<0.05),the higher the serum concentration of CA125 The prognosis of early threatened abortion patients is worse.5.According to the ROC curve,serum progesterone ? 24.265 ng/ml was selected as the index of poor prognosis in patients with early threatened abortion.The sensitivity and specificity were 0.809 and 0.200,respectively;serum HCG ? 12962 ml U/ml was selected as early threatened abortion patients.The indexes with poor prognosis were sensitivity and specificity of 0.979 and0.867,respectively.Serum CA125 ? 29 U/ml was selected as the index of poor prognosis in patients with early threatened abortion.The sensitivity and specificity were 0.933 and 0.862,respectively.Research Conclusion: 1.The pregnancy outcomes of patients with early threatened abortion were correlated with serum HCG,progesterone,and CA125 levels(P<0.05).2.The levels of serum HCG,progesterone and CA125 in normal pregnant women,early threatened abortion patients and early threatened abortion patients were significantly different between the successful group and the inevitable miscarriage group.(P<0.05).3.The serum progesterone?24.265ng/ml,serum HCG?12962m IU/ml,and serum CA125?29U/ml in threatened abortion patients indicated that the high risk of adverse pregnancy outcome. |
