Objective: To observe the serum levels of ?-HCG,PROG,E2,CA125 and cytokines in pregnant women with threatened abortion,and to explore their diagnostic value for threatened abortion and predictive value for pregnancy outcome.Methods: 120 cases of threatened abortion pregnant women(observation group)and 100 cases of healthy early pregnant women(control group)were selected as the research objects.The pregnancy outcome of observation group was followed up.According to the pregnancy outcome,observation group was divided into continuous pregnancy group and pregnancy failure group.Serum levels of?-HCG,PROG,E2,CA125 and cytokines were detected in all subjects,and the differences between groups were compared.The diagnostic and prognostic value of serum beta-HCG,PROG,E2 and CA125 in threatened abortion was analyzed by ROC.Results:(1)Compared with the control group,the serum levels of beta-h CG?-h CG[(4902.51±253.61)U/L VS(6421.57±532.58)U/L] ?PROG[(24.13±6.59)ng/m L VS(39.25±12.57)ng/m L]?E2[(268.24±36.95)pg/m L VS(355.61±68.25)pg/m L] in the observation group were lower than those in the control group(P < 0.05).The level of CA125 in the observation group was higher than that in the control group [(32.14±8.52)IU/m L VS(18.69±4.33)IU/m L],and the difference was statistically significant(P<0.05).The levels of?-h CG[(5121.25±116.25)U/L VS(4526.15±110.25)U/L]?PROG[(26.35±7.59)ng/m L VS(18.56±4.68)ng/m L]?E2[(295.12±43.52)pg/m L VS(221.03±35.16)pg/m L] in the continued pregnancy group were higher than those in the failed pregnancy group(P<0.05).(2)Beta-HCG,PROG,E2 and CA125 all have high diagnostic and prognostic value for threatened abortion.Among them,beta-h CG and PROG have high diagnostic and prognostic value for threatened abortion.The area under curve(AUC)was 0.872(95% CI: 0.768-0.976,P = 0.000),0.869(95% CI: 0.765-0.973,P = 0.000),and the predictive effect of beta-h CG and E2 for threatened abortion pregnancy outcomes was higher,AUC was 0.922,respectively.(95% CI: 0.853-0.991,P = 0.000),0.872(95% CI: 0.768-0.977,P = 0.000).(3)The sensitivity,specificity,positive predictive value and negative predictive value were 85.64%,94.71%,88.49% and 95.84% respectively.At the same time,the sensitivity,specificity,positive predictive value and negative predictive value were 89.71%,85.24%,94.17% and 89.67% respectively,which were better than those of single serum marker.Post-prediction.(3)The levels of serum IL-6,IL-8 and IL-10 in the observation group were higher than those in the control group,and the levels of TNF-alpha were lower than those in the control group(P < 0.05).The levels of serum IL-6,IL-8 and IL-10 in the continued pregnancy group were lower than those in the failed pregnancy group,and the levels of TNF-alpha were higher than those in the control group(P < 0.05).Conclusion: Serum levels of beta-h CG,PROG,E2 and CA125 can be used to diagnose threatened abortion and effectively predict the pregnancy outcome of threatened abortion patients.Clinical monitoring of beta-h CG changes should be emphasized for pregnant women with threatened abortion,combined with E2,PROG,CA125,cytokine detection,in order to make the best diagnosis and treatment plan.
