Objectives We demonstrate the types of inflammatory cells infiltrating in the lesional skin of pityriasis rosea(PR),and explore whether T cells mediated immunity is involved in the pathogenesis of PR or not.Methods 1.The biopsies were taken from the lesional skins of 35 cases of patients diagnosed as PR.The patient’s gender,age,address,telephone number,the duration of disease,the distribution of skin rashes and the extent of itching were recorded in the first clinical visit.The skin biopsies were performed from 35 PR patients in the out-patient operating room after the patient’s informed consent.The specimen were prepared to paraffin sections,then examined by routine immunohistochemistry procedures using monoclonal antibody directing against CD3,CD4,CD8,CD20 and CD45 RO and horseradish peroxidase labeled goat antihuman anibody.After the first clinical visit,35 PR patients were followed up by telephone to 4 weeks after the lesion completely subsided.2.Each section was observed in ten high power fields to take the average score.The positive section was determined by the ratio and staining intensity of positive inflammatory cells.The score of staining intensity was rated 0(negative),1(faint yellow),2(brown)and 3(tan-colored),while the score of the ratio of positive cells was rated 0(negative),1(< 10%),2(10%-25%),3(26%-50%),4(51%-75%)and 5(75%-100%).The rate and staining intensity of the positive inflammatory cells were adopted by the scoring of immunoreaction scores proposed by Remmele and Stegner.The staining of section is determined to be positive if the final score was≥3.Then the res μlts were analyzed statistically used SPSS16.0 software.Res μlts 1.From December 2013 to April 2015,35 cases of pityriasis were collected,including 19 males and 16 females.The age of patients ranged form 17 years old to 53 years old,and the mean age was 29.06± 9.43 years old.The duration of disease ranged from 1 day to 6 months,and the average duration was 29.03 days;Patients were followed up by telephone.The mean remission time of disease was 67.54 ±44.86 days(ranged from 21 days to 240 days).2.35 patients with PR gave priority in the staining of CD3,CD4,CD8 and CD45 R,and the positive ratios were 54.29%(19/35),69.7%(23/33),40%(14/35)and 79.41%(27/34)with statistical significance(P < 0.05).The tissues were dropped from the slices during the preparation of CD4 staining in two cases and CD45 RO staining in one case.The mean score of positive CD3,CD4,CD8,and CD45 RO staining was respectively 3.74±3.88,5.67± 4.40,2.94±3.42 and 7.68±4.33 in these PR patients(P<0.001,F =9.558).The average score of CD45 RO staining was the highest.However,the staining of CD20 was completely negative among all samples.CD3,CD4,CD8 and CD45 RO staining were not statistically significant(P > 0.05)between PR patients with different age and different disease duration(greater than 1 week or less than 1 week).The mean score of CD3 staining(4.90±4.21)in the patients with remission time ≤60 days was higher than that(2.00±2.5)in the patients with remission time >60 days(P<0.05),whereas no statistical difference in the mean score of CD4,CD8 and CD45 RO staining.The inflammatory cells were mainly infiltrated in the dermis.but in the study,the epidermal infiltration of inflammatory cells with positive CD3,CD4,CD8 or CD45 RO staining was observed in 5 patients.Conclusions 1.The T cells predominated in the infiltrate of lesional skin of PR,and none of B cells was observed.2.The T cells mediated immunity was likely to play some role in the pathogenesis of PR.
