Objective To study the expression of hypoxia inducible factor-1alpha(HIF-1alpha),tumor necrosis factor-alpha(TNF-alpha),matrix metalloproteinase-1(MMP-1)and the amount of collagen type I in meniscus cartilages in knee osteoarthritis(KOA).To discuss the correlation between them and the meniscus degeneration score.To probe the effect of them on meniscus degeneration.Methods Collect the meniscus of the patients in the Department of orthopedics,Anqing Hospital Affiliated to Anhui Medical University from January 2016 to January2017.The patients with KOA undergoing total knee joint replacement were served as the experimental group(16 cases),and the patients undergoing amputation for destructive injury of lower limbs were served as the control group(5 cases).HE staining and histological scoring were performed to evaluate meniscus degeneration score of each group.Immunohistochemical staining was conducted to calculate and analyze the rate of chondrocytes with positive expression of HIF-1alpha,TNF-alpha,MMP-1 and collagen type ? positive grey value.Results The meniscus degeneration score of the experimental group(5.813±1.642)were significantly higher than those of the control group(0.600±0.548)(P<0.05).The rate of chondrocytes with positive expression of HIF-1alpha of the experimental group(17.547±8.217)% were significantly higher than those of the control group(0.852±0.827)%(P<0.05).The rate of chondrocytes with positive expression of TNF-alpha of the experimental group(24.601±11.331)% were significantly higher than those of the control group(1.358±0.825)%(P<0.05).The rate of chondrocytes with positive expression of MMP-1 of the experimental group(14.563±8.303)% were significantly higher than those of the control group(1.638±2.079)%(P<0.05).The collagen type ? positive grey value of the experimental group(0.064±0.033)were significantly lower than those of the control group(0.192±0.390)(P<0.05).There is a correlation between the meniscus degeneration score and the rate of chondrocytes with positive expression of HIF-1alpha(R=0.663,P<0.05),TNF-alpha(R=0.798,P<0.05),MMP-1(R=0.642,P<0.05)and collagen type ? positive grey value(R=-0.844,P <0.05)Conclusions 1.The knee of the patients with KOA are usually accompanied by meniscus degeneration;2.The expression level of HIF-1alpha,TNF-alpha and MMP-1 in the meniscus of degenerative meniscus is significantly higher than that of normal meniscus;3.The meniscus degeneration score is correlated with the expression level of HIF-1alpha,TNF-alpha and MMP-1;4.The content of type I collagen in the degenerative meniscus is significantly less than that of normal meniscus,and the meniscus degeneration score is correlated with the content of type I collagen.
