Tuberculous wound occurs when the mycobacterium tuberculosis invade skin,which can not heal naturally.The endless treatment cycle from months to years means a great physical and psychological burden to patients and reduces the patient's quality of life seriously.How to promote the healing of tuberculous wound becomes a difficult problem in clinical treatment of tuberculous wound.Animal model is the basic of the study in disease occurrence and development.So far,guinea pig,New Zealand rabbit and zebrafish have been found to form tuberculous wound.However,the mature and stable animal model of tuberculous wound which can be uesd for therapy research in tuberculous wound has not yet been established.Basic fibroblast growth factor(b FGF),known as a kind of multifunctional growth factor,is widely used in pressure sores,diabetic foot,radioactive ulcer,which is effective at promoting the healing of refractory wounds.The effect of b FGF on tuberculous wound has not been reported at home and abroad.The clinical study of antituberculosis drug used for tuberculous wound treatment has been reported,while the specific effect and mechanism still need to be further researched.To investigate the effects and possible mechanisms of basic fibroblast growth factor and antituberculosis drug on healing of wound due to Mycobacterium tuberculosis infection.In this study,BCG was injected into the back of the 32 complete Freund's adjuvant-immuned New Zealandrabbits by intradermal route and the model of tuberculosis wound in New Zealand rabbit was established.The rabbits were divided into growth factor group(Group A),antituberculosis drug group(Group B),growth factor union antituberculosis drug group(Group C)and control group(Group D)according to the random number table,with 8 rabbits in each group.After a complete debridement of the wounds,each group was treated with the corresponding drugs with dressing change of once every two days.Immediately after surgery and on post surgery day(PSD)7,14,21,28,the general observation and calculation on wound healing rate were conducted.Record wound healing time.The tissue samples of unhealed wound were collected for observation of histomorphological change with HE staining and Masson staining,observation of the number of microvessels with CD31 antibody immunohistochemical staining,and determination of the level of hydroxyproline by enzyme-linked immunosorbent assay.Data were processed with one-way analysis of variance,LSD-t test.The results show that:(1)On PSD7,14,21 and 28,the wound healing rates of rabbits in Group A,Group B,Group C were significantly higher than the wound healing rate of rabbits in Group D.On PSD14,21 and 28,the wound healing rates of rabbits in Group A and Group C were significantly higher than the wound healing rate of rabbits in Group B.The wound healing time of Group A,Group B,Group C were(29.50±2.45)d,(31.63±1.30)d and(28.13±0.99)d respectively,significantly shorter than the wound healing time of Group D[(35.63 ±2.33)d,P values below 0.05].The wound healing time of Group A and Group C were significantly shorter than the wound healing time of Group B,P values below 0.05.(2)On PSD21,the tissue samples of wounds in Group A and Group C showed complete tissue structure and relatively clear histological hierarchy with HE staining and Masson staining.Well reorganized collagen fibers were observed in the healed wound tissue of rabbits in Group A and Group C.(3)On PSD21,the number of microvessels in healed wound tissue inGroup A,Group B,Group C were(31.63±1.19),(27.53±1.26)and(32.84±1.60)respectively,significantly higher than the number of Group D(22.30±1.68),P values below 0.05.The number of microvessels in healed wound tissue in Group A and Group C were significantly higher than the number of Group B,P values below 0.05.(4)On PSD14 and 21,the level of hydroxyproline in healed wound tissue in Group A,Group B,Group C were significantly higher than the level of Group D,P values below 0.05.The level of hydroxyproline in healed wound tissue in Group A and Group C were significantly higher than the level of Group B,P values below 0.05.The above results suggest that the model of tuberculosis wound in New Zealand rabbit can be used as a reliable animal model for the study of human tuberculous wound.Antituberculosis drug can be used to promote healing of tuberculosis wound in New Zealand rabbit after a complete debridement of wound and to improve the quality of wound healing.Basic fibroblast growth factor can be used solely or combined with antituberculosis drug to promote healing of tuberculosis wound in New Zealand rabbit after a complete debridement of wound,which is superior to the antituberculosis drug group.
