Clinical Features Of The Elderly Patients With Type 2 Diabetes Mellitus And OSAHS

Abstract/Summary:
Objective:To investigate the clinical features of the elderly patients with type 2 diabetes mellitus and obstructive sleep apnea-hypopnea syndrome(OSAHS).Method:Retrospectively analyzed the clinical data of 115 patients diagnosed with type 2 diabetes and performing sleep-respiratory monitoring in the Department of Geriatric Endocrinology of the First Affiliated Hospital of Kunming Medical University from September 2017 to March 2018.Among them,there were 82 cases of type 2 diabetes with OSAHS,and 33 cases of type 2 diabetes without OSAHS.According to age,the patients suffering from type 2 diabetes mellitus combined with OSAHS were divided into the elderly group(51 cases)and the young and middle-aged group(31 cases),and their clinical features during the hospital stay were collected and statistically analyzed.Results:①The proportion of OSAHS in patients with type 2 diabetes was 71.3%.Among them,51 were elderly people,accounting for 62.2%,and 31 were young and middle-aged,accounting for 37.8%.There were 58 males,accounting for 70.7%,and 24 females,accounting for 29.3%.② Among patients suffering from type 2 diabetes mellitus with OSAHS,34 were mild,accounting for 41.4%,29 were moderate,accounting for 35.4%,and 19 were severe,accounting for 23.2%.Among the elderly patients,25 were mild,accounting for 30.5%,14 were moderate,accounting for 17.1%,and 12 were severe,accounting for 14.6%;Among the young and middle-aged group,9 were mild,accounting for 11%,15 were moderate,accounting for 18.3%,and 7 were severe,accounting for 8.5%.③ The proportion of LSaO2≤80%,coronary heart disease,smoking and alcohol consumption in patients with type 2 diabetes mellitus combined with OSAHS was higher than that in patients suffering from type 2 diabetes mellitus without OSAHS,which was statistically significant(P<0.05).④In patients with type 2 diabetes mellitus combined with OSAHS,the red blood cells,hemoglobin,total cholesterol,triglycerides,glomerular filtration rate,fasting blood glucose,and MSaO2 were less in the elderly group than in the young and middle-aged group,and the difference was statistically significant(P<0.05).⑤ In patients with type 2 diabetes mellitus combined with OSAHS,AHI was negatively correlated with C-peptide and C-peptide at postprandial 2 hours(r=-0.230,P=0.038;r=-0.301,P=0.032),and there was a positive correlation between LSaO2 and fasting blood glucose and postprandial blood glucose for 2 hours(r=0.321,P=0.003;r-0.262,P=0.011)which was negatively correlated with C-peptide at 2 hours postprandial(r=-0.251,P=0.023).Conclusion:①The proportion of patients with type 2 diabetes mellitus combined with OSAHS is high,and patients with OSAHS are more common in older adults and males.②The severity of patients with OSAHS is mainly mild,among them,older people are mild,and young and middle-aged people tend to be moderate.③The proportion of patients suffering from type 2 diabetes mellitus combined with OSAHS with coronary heart disease is higher,Smoking and drinkers are easy to incorporate OSAHS.④Type 2 diabetes combined with OSAHS may worsen pancreatic β-cell function.
