Objectives:This study analyzed the clinical manifestations and laboratory examinations of primary Sj?gren's syndrome(pSS)combined with in extraglandular organ involvement,and explored the relationship between the characteristics of different organs involved and the relationship between the number of antibodies and the number of organs involved.The knowledge of the disease provides the basis for clinicians to find out the involvement of extraglandular organs.Methods:We retrospectively analyzed the clinical data of 176 cases of pSS hospitalized in our hospital from January 2014 to November 2017.We analyzed the course,age,clinical manifestations,and immunological parameters of patients with combined extraglandular organ involvement,using statistical methods.Related conclusions.Results:Among the 176 cases of pSS,148 cases(84.1%)were involved in the involvement of extraglandular organs of the glands,including 108 cases(61.4%)affected by the blood system,22 cases(12.5%)of liver involvement,27 cases(15.3%)of involvement of the kidney,and skin involvement,eleven patients(6.3%),joint involvement in 62(35.2%),pulmonary interstitial involvement in 22(12.5%),and nervous system involvement in 3(1.7%).2.In leukocytopenia patients(68.5%),patients with hematologic involvement were more likely to suffer from blood system involvement than those with unrelated blood system involvement.Patients with hematologic involvement were more likely to have anti-SSA antibodies,anti-SSB antibodies,positive particle,and decreased C3 incidence.The difference between the two groups was statistically significant(P<0.05).3.The age of patients with pSS and pulmonary interstitial involvement was greater than that of patients without pulmonary interstitial involvement.The difference between the two groups was statistically significant(P<0.05).The positive rate of cytoplasmic particles was significantly higher than that of uncombined patients.Statistically significant(P<0.05),multivariate analysis suggested that age,positive cytoplasmic granules,and elevated globulin were risk factors for pulmonary interstitial involvement.4.The renal involvement of pSS combined with renal tubular acidosis in 15 cases(55.6%),glomerular lesions in 5 cases(18.5%),renal insufficiency in 7 cases(25.9%).Patients with pSS and renal involvement were prone to granules positive,C3 decreased,CRP increased,and IgA increased.There was a statistically significant difference between the two groups(P<0.05).The level of C3 was lower than that of the patients without renal involvement.There were significant differences between the two groups.Statistical significance(P<0.05),globulin levels were higher than those without renal involvement,and the difference between the two groups was statistically significant(P<0.05).Multivariate analysis suggested that elevated CRP and decreased C3 were risk factors for pSS and renal involvement.5.The average age of pSS patients with liver involvement was greater than that of patients without liver involvement.There was significant difference between the two groups(P<0.05).Patients with liver involvement were prone to homogeneous positive,elevated CRP,and elevated IgM.The level of globulin was significantly higher than that of the unaffected liver group.There was a significant difference between the two groups(P<0.05).6.Patients with pSS and joint involvement were mainly characterized by joint swelling and pain.The affected joints were mainly knee joint(56.5%),proximal interphalangeal joint(37.1%),metacarpophalangeal joint(23.2%),and wrist joint and ankle joint.Shoulder joints,elbow joints may also be involved.7.The average number of in vitro organ involvement of pSS combined glands was 1.4.The more antibodies,the more the number of glandular organ involvement(r=0.258,P=0.001).Conclusion:1.The incidence of extraglandular organ involvement in pSS was higher,mainly due to involvement of the blood system,followed by joint involvement.The number of pSS antibodies was positively correlated with the number of extraglandular organs.2.The leukocytopenia is predominantly involved in the blood system of pSS.Anti-SSA antibody,anti-SSB antibody,positive granules,and decreased C3 are significant in predicting the involvement of blood system.3.Age,positive cytoplasmic granules,and elevated globulin were risk factors for interstitial lung involvement of pSS.4.The renal involvement of pSS is mainly renal tubular acidosis,CRP increase,C3 decline is a risk factor associated with renal involvement.5.Patients with liver involvement of pSS had older age,posive homogeneous type,higher CRP,higher IgM,elevated globulin are associated with liver involvement.
