Objective:To analysis and summary the situation and prevention measures of sedated upper gastrointestinal endoscopy adverse events.Methods:To review relative materials and processes of patients?s sedated upper gastrointestinal endoscopy in hospital,including age,gender,smoking,body mass index,hypertension,coronary heart disease,diabetes mellitus,cerebrovascular disease,gastric surgery methods from Jan.1 of 2008 to Dec.31 of 2017,the sum of 10 years.80 elderly critically ill patients undergoing gastroscopy were randomly selected and divided into two groups:sedated upper gastrointestinal endoscopy group and transnasal gastroscopy group,with 40 patients in each group.Observe and record adverse reactions during the examination.Results:The adverse events of sedated upper gastrointestinal endoscopy mostly happens on the male in the elderly.One case dead in older men.The cause of death is presumed that the esophageal"polyp"pedicle folded back to the back endoscopy throat airway obstruction caused by suffocation.One case experienced fleeting ischemic attack.One case experienced Apnea which cause rib fracture after CPR.A total of two cases experienced aspiration pneumonia after the rescue by ventilator,which was due to aspiration.A total of 4576 cases experienced SpO2 decreased to30%or even 0,which rehabilitated by oxygen masks and artificial respiratory.Two cases is diarrhea.One case experienced burst infarction in underwent endoscopy,while one day ago the ECG is normal.One case evoked hysteria.The 80 elderly patients were equally divided into two groups:40 in the sedated upper gastrointestinal endoscopy group,40 in the transnasal gastroscopy group,There were 15 adverse events in the sedated upper gastrointestinal endoscopy group.No adverse reactions occurred in the transnasal gastroscopy group.Conclusion:The adverse events of sedated upper gastrointestinal endoscopy are more concentrated in middle-aged males,with men smoking,diabetes,hypertension,obesity,coronary heart disease are closely related.Patients with aspiration pneumonia caused by inhalation occurred in the gastric retention,gastric cancer postoperative.Aspiration,pneumonia and other adverse events can be avoided if eliminate gastric retention by examination preoperatively,gastric ultrasound,CT and other tests.Adverse events can be avoided by carefully evaluate preoperative condition,improve serum creatine kinase,lung function and other tests.Transnasal gastroscopy in the elderly is relatively safe and efficient,and examination of elderly critically ill patients with less complications in the transnasal gastroscopy group than in the sedated upper gastrointestinal endoscopy group.
