Objective:1.To make a epidemiological investigation of HIV / AIDS patients with perianal condyloma acuminata and HPV-DNA typing were performed.2.To investigate the efficacy of high frequency electric knife combined with imiquimod in the treatment of HIV / AIDS combined with perianal condyloma acuminatum.Methods:1.A total of 125 cases of HIV / AIDS population who have sex with men from December 2013 to June 2016 were screening in our hospital.All the selected patients were investigated for the factors affecting the incidence of perianal condyloma acuminatum.The total number of male sexual partners,the number of male partners in the past six months,the frequency of sex in the past six months,the frequency of condom use and so on were screening and detection of HPV-DNA and typing by DNA dot hybridization test.2.A total of 28 patients who were diagnosed with HIV / AIDS combined with perianal condyloma were selected and randomly divided into study group and control group(14 cases each).The control group was divided into control group A and B,7 cases.The patients in the study group were treated with high frequency electric knife combined with imiquimod,the control group A was treated with high frequency electric knife,and the control group B was treated with liquid nitrogen frozen treatment.The symptoms of remission were observed after treatment of HIV / AIDS combined with perianal condyloma.The curative effect and incidence of adverse reactions were also compared.Results:1.From the 125 patients,61(48.0%)had no significant changes in the perianal skin,28(22.40%)had typical condyloma in the perianum,and 36(28.80%)had suspected perianal suspicion skin lesions.In 28 cases,26 cases and 33 cases of HPV-DNA positive infection,the positive rates were 100%,72.22% and 54.10% respectively in HIV / AIDS patients with typical condyloma acuminata,perianal suspicious lesions and no obvious lesions.From the 87 patients with HPV-DNA positive,59(67.82%)had two or more HPV-DNA positive infections.In HPV-DNA typing test,the infection rates of HPV16,18,6 and 11 were relatively high.However,the infection rates of HPV16,18,6 and 11 were not statistically different between the three different types of HIV / AIDS patients(X2 = 1.052,0.983,0.892,P> 0.05).This study found no independent risk factors for the occurrence of perianal condyloma acuminata in HIV / AIDS patients.2.28 cases of confirmed patients were followed up for 6 months,9 cases recovered in the study group,but in the control group A 2 cases recovered,and 3 cases recovered in the control group B recovered.The number of cured case in the study group was significantly higher than the control group(P <0.001).The effective rate in the study group was 92.86%,which was higher than that of the 85.71% of control group,but there was no significant difference(P=0.275).There was a recurrence rate(14.29%)in the study group and 4 patients(28.57%)in the control group.There was a significant statistical difference between the two groups.The recurrence rate was significantly lower in the study group than in the control group(P=0.032).No statistical difference was observed between the two groups(P = 0.733).Conclusions:1.In patients with HIV / AIDS patients with perianal condyloma,the positive rate of HPV-DNA mixed infection is high.HPV-6,11,16,18 is a common HPV-DNA typing.however,our study found no risk factors for the occurrence of perianal condyloma in HIV / AIDS patients.2.The use of high-frequency electric knife plus imiquimod treatment of HIV / AIDS combined with perianal condyloma is better than liquid nitrogen frozen or high frequency electric knife treatment,which is worthy of clinical promotion.
