Clinical Significance Of Urine PCA3 Detection In Early Diagnosis And Screening Of Prostate Cancer

Objective: To study the expression of PCA3 and PSA in urine collected after prostate massage,and to clarify the clinical significance of PCA3 Score in the early diagnosis and screening of prostate cancer.Methods: After prostate puncture,50 patients with prostate cancer were clearly identified as group A;60 patients with prostate hyperplasia were group B;54 cases of prostatitis diagnosed by prostatic fluid examination were group C;29 cases of healthy male volunteers who were not abnormal after physical examination were group D;The first urine was collected from each group of patients after prostate massage and the cells were isolated.The expression of PCA3 and PSA in urine was detected by fluorescence PCR and the ratio of PCA3 gene expression to PSA gene expression was calculated.Results:(1)According to ROC curve analysis,ROC showed that the area under the PCA3 score curve was 0.831.When PCA3 Score cutoff was 40.1518,the diagnostic sensitivity was 82.4% and the specificity was 77.8%.For ROC statistical analysis of serum PSA levels,the area under the PSA curve was 0.66,the difference between the two idicators was significant.(2)The positive rates of PCA3 score in urine of group B,C and D were 8.70%,5.56%,0%,while the positive rate of PCA3 score in urine of group A was 84.21%.The difference between group A and group B,group C and group D was statistically significant(P<0.05).(3)Among the 110 prostate puncture results,the sensitivity and specificity of urinary PCA3 score were 75.00% and 85.14%,respectively.The sensitivity and specificity of serum t PSA were 50.75% and 62.79%,respectively.Conclusion: Urine PCA3 is highly expressed in prostate cancer after prostate massage.It has higher sensitivity and more specificity compared with PSA.It will significantly reduce the number of prostate biopsy.The detection of PCA3 in urine after prostate massage has important clinical significance in the diagnosis and screening for early prostate cancer.PCA3 score can be used as an indicator for early detection of prostate cancer.
