ObjectiveTo investigate the clinical effects of practical psychological support through WeChat platform combined with biofeedback therapy on patients with chronic constipation.MethodSixty cases of patients with chronic functional constipation were selected in the department of Zhujiang hospital,Southern Medical University,whether outpatients or inpatients.The self-evaluation of anxiety scale(SAS)and the depression self-evaluation scale(SDS)were applied to assess the psychological conditions,and the patient assessment of constipation quality of life scale(PAC-QOL)was used to evaluate the impact on the daily life.Base on the score of the mental state and the therapeutic protocols,all the patients were divided into three groups,the control group,the non-psychological care group and the psychological care group respectively.All the three groups were then received biofeedback treatment,and the psychological care group received psychological support through our WeChat support platform at the same time.All the changes were compared before and after the treatment.The difference among the three group were also analyzed as well.T-test was performed for comparison between before and after treatment,and the one-way analysis variance was performed for comparison among multiple group.Result1.The mental state of the patients with constipationOf the 50 patients,the score of S AS and SDS were abnormal in 32(64%)and 31(74%)patient respectively.The decreasing score of the SAS and SDS of the three group were noted following the treatment.There were no statistically significant differences in the scores between the non-psychological care group and the psychological care group before the treatment.Compared with the non-psychological care group[SAS(56.35±5.18),SDS(51.18±4.876)],the psychological care group[SAS(44.35±6.99),SDS(42.12±6.508)]was significantly decreased after the treatment(P all<0.05).2.The life quality of the patients with constipationAfter the treatment,The scores of PAC-QOL of the three groups were lower than those before the treatment[(45.44±10.25)vs(58.75±14.60),(55.94±10.25)vs(64.82±11.99),(46.59±9.612)vs(67.81 ± 13.77),Pall<0.05].There were no statistically significant differences in the scores among the three group before the treatment.After the treatment,the PAC-QOL scores of the non-psychological care group was significantly higher compared with the control group(P<0.05).Compared with the non-psychological care group,the PAC-QOL scores of the psychological care group was significantly decreased(P<0.05).3.Parameters related to the anorectal manometryAfter the treatment,the increased rectal pressure during defecation(IRP)of the three group were both increased than those before[76.08±16.30)mmHg vs(44.18±9.53)mmHg,(60.48±1563)mmHg vs(46.41±19.98)mmHg,(73.41±23.02)mmHg vs(41.56±20.70)mmHg,P all<0.05].After the treatment,the maximal rectal pressure(MRP)during defecation of the three group were both increased than those before[(93.35±20.35)mmHg vs(67.78±21.79)mmHg,(79.55±14.05)mmHg vs(66.38±19.02)mmHg,(100.76±22.64)mmHg vs(62.80±26.38)mmHg,P all<0.05].Meanwhile,the minimum anal loose pressure during defecation(MALP)of the three group was decreased than those before[(32.58±8.29)mmHg vs(64.46±11.66)mmHg,(43.87±17.28)mmHg vs(67.55±21.70)mmHg,(27.26±17.39)mmHg vs(59.12±21.49)mmHg,P all<0.05].There were no statistically significant differences in the above parameter among the three group before the treatment.After the treatment,the IRP and MRP of the non-psychological care group were lower while the MRLP was higher when compared with the control group(P<0.05).Compared with the non-psychological care group,the IRP and MRP of the psychological care group were both significantly increased compared with the non-psychological care group(P<0.05)while the MRLP was decreased(P<0.05).There were no statistically significant differences in the following parameter,the anal resting pressure,maximal anal squeeze pressure and the anal squeeze duration time,among the three group before and after the treatment(P>0.05).4.Clinical efficacy and satisfaction degree4.1 After treatment,the Bristol stool form cale scores(BSS)of the psychological support group was statistically higher compared those before.(P<0.05)while there were no statistically significant differences of the control group and the non-psychological support group before and after the treatment(P>0.05).There was no significant difference of BSS among the three groups before the treatment(P>0.05).Compared with the non-psychological care group,the BSS of the psychological care group was significantly higher(P<0.05).4.2 After the treatment,the balloon expulsion time(BET)of the three group was decreased than those before[(7.63 ± 2.41)S vs(11.69±2.36)S,(9.76±2.75)S vs(11.71±2.90)S,(7.12±2.44)S vs(11.53±2.50)S,P all<0.05].After the treatment,the BET of the non-psychological care group was significantly longer compared with the control group(P<0.05).Compared with the non-psychological care group,the BET of the psychological care group was significantly shorter(P<0.05).4.3 After the treatment,the scores of the patient treatment satisfaction of the three group were[(3.56±0.72),(3.00±0.71),(3.94±1.08),P all<0.05].The psychological care group was significantly higher compared with the non-psychological care group(P<0.05).Ps:All the above data of the three group are presented as the following order:the control group,the non-psychological care group and the psychological care group respectively.ConclusionThe curative effect of biofeedback therapy in patients with chronic functional constipation is affirmative.Most patients suffered from long-term difficulty in defecation experience different degrees of anxiety and depression.Practical psychological support combined with biofeedback therapy can be more effective to improve not only clinical symptoms and mental conditions,but also therapeutic effect of biofeedback therapy on patients with chronic constipation.
