Effects Of Liraglutide On Levels Of Serum Inflammatory Factors In Patients With Type 2 Diabetes Mellitus

Objective:It is to observe the effects of liraglutide on serum soluble CD163(sCD163),interleukin-1?(IL-1?),interleukin-6(IL-6),tumor necrosis factor a(TNF-a),hypersensitive C reactive protein(hs-CRP),leptin(LP),adiponectin(APN)and other indexes levels in patients with type 2 diabetes mellitus(T2DM).Methods:42 poorly controlled blood glucose subjects with T2DM treated with oral sulfonylurea plus metformin were enrolled in endocrinology department of our hospital from January 2016 to June 2017.They were randomly divided into 2 groups,which combined with liraglutide(liraglutide group n=20)or insulin glargine(insulin glargine group n=22)for 12 weeks,respectively.The goal of blood glucose control is fasting blood glucose(FBG)4.4-7.0mmol/L,2 hours postprandial blood glucose(2hPG)<10mmol/L,HbAlc<7%.During the treatment period,the original anti-hypertension and lipid-lowering plan remained unchanged.The change of general information,serum inflammatory factors,glycometabolism indexes and lipid metabolism indexes in the two groups before and after treatment were analyzed and compared.Results:After liraglutide treatment,the body weight,Body mass index(BMI),waist circumference,fasting blood glucose,2 hours postprandial blood glucose,HbA1c and HOMA-IR in T2DM patients were all lower than those before,serum inflammatoryfactors,sCD163(45.10±13.25 vs 36.07±11.58,P?0.001),IL-1?(112.62± 4.84vs84.08 ± 4.64,P<0.001),IL-6(508.06 ± 130.84 vs 288.92 ±108.63,P<0.001),TNF-?(144.19±31.13 vs 110.25±27.40,P<0.001),hs-CRP(4.71 ±2.16 vs 4.27 ± 2.01,P<0.001)in T2DM patients were all lower than those before,fasting insulin(FINS),C-peptid(C-P)and serum anti inflammatory factor APN(9.69 ± 0.22 vs 11.85 ±0.22,P<0.001)were higher than before treatmentthe differences was statistically significant(P<0.05).After insulin glargine treatment,blood glucose and HbAlc of T2DM patients were lower than before treatment,FINS and C-P increased compared with those before treatment,the difference was statistically significant(P<0.05).Conclusion:liraglutide and Insulin glargine has good hypoglycemic effect.But compared with insulin glargine,liraglutide has the advantage of reducing BMI,serum inflammatory factor(sCD163?IL-1?.IL-6?TNF-??hs-CRP)and increase the level of serum anti inflammatory factor APN in T2DM patients.
