Objectives The analysis of fundus arteriosclerosis,carotid artery stenosis,and the Oxfordshire Community Stroke Project(OCSP)classification were performed in patients with acute cerebral infarction.To investigate the correlation between fundus arteriosclerosis,carotid artery stenosis and OCSP classification in patients with acute cerebral infarction.Methods Among the 215 patients with acute cerebral infarction who were hospitalized from October 2016 to December 2017 in a hospital affiliated to North China University of Science and Technology,all of them underwent fundus photography and neck vascular ultrasonography.The retinal artery morphological characteristics and retinopathy were observed with fundus photography.According to the fundus photographic results,the patients with acute cerebral infarction with fundus arteriosclerosis were selected as the case group(fundus arteriosclerosis group),and the patients with no fundus arteriosclerosis were the control group(non-fundus arteriosclerosis).The fundus arteriosclerosis was graded.Judging whether there is carotid artery stenosis based on results of carotid ultrasound.Severe grading of the stenotic carotid artery.The carotid artery stenosis between the fundus atherosclerosis group and the non-fundal atherosclerosis group was compared,and the correlation between fundus arteriosclerosis and artery stenosis was analyzed.OCSP classification was based on the clinical manifestations of maximal neurological deficit in patients with acute cerebral infarction.They were divided into Total Anterior Circulation Infarct(TACI),Partial Anterior Circulation Infarct(PACI),Posterior Circulation Infarct(POCI)and Lacunar Infarct(LACI).The distribution of fundus arteriosclerosis of each subtype and the correlation of fundus arteriosclerosis and OCSP classification were analyzed.Results 1 Of the 215 patients with acute cerebral infarction,40(18.6%)had no fundus arteriosclerosis and 175(81.4%)had arteriosclerosis.Among the fundus arteriosclerosis group,53 cases(30.3%)were grade I hardened,29 cases(16.6%)were grade II hardened,93 cases(53.1%)were grade III hardened,and none grade IV hardened.2 1)Carotid stenosis was compared between the fundus arteriosclerosis and the non-fundus arteriosclerosis group.The proportion of carotid artery stenosis in the fundus arteriosclerosis group was significantly higher than that in the non-ocular fundus arteriosclerosis group.(P<0.05).In patients with acute cerebral infarction,fundus arteriosclerosis was associated with carotid artery stenosis.In patients with with carotid stenosis,the risk of retinal arteriosclerosis was doubled(OR=2.083,95% CI 1.035 ~ 4.191).Multivariate logistic regression analysis showed that after controlling for other influencing factors,retinal arteriosclerosis was still associated with carotid stenosis in patients with acute cerebral infarction(OR=2.435,95%CI 1.053 ~ 5.632).2)Carotid artery stenosis was graded in patients with acute cerebral infarction at each grade of fundus arteriosclerosis.Spearman rank correlation test showed that there was a positive correlation between retinal arteriosclerosis grade and carotid stenosis(r=0.242,P<0.05).3 1)OCSP classification was performed on patients with acute cerebral infarction.The distribution of the arteriosclerotic sclerosis in each subtype was analyzed.Chi-square test showed that the OCSP subtypes were not all the same in visual atherosclerosis(P<0.001).Fundus arteriosclerosis was associated with PACI type and LACI type in patients with acute cerebral infarction(PACI: OR=15.583,95%CI 4.735~51.282;LACI: OR=8.433,95%CI2.522~28.200).2)Grade OCSP subtypes fundus arteriosclerosis.Kruskal-wallis H test showed that the degree of arteriosclerosis was not the same in each subtype(P <0.05).The Nemenyi test showed that the degree of fundus arteriosclerosis was more severe in the PACI type(PPACI vs LACI<0.05).Conclusions 1 In patients with acute cerebral infarction,fundus arteriosclerosis is related to carotid artery stenosis.There is a positive correlation between the degree of fundus arteriosclerosis and carotid stenosis.2 In patients with acute cerebral infarction,arteriosclerosis is related to the PACI type and LACI type in OCSP subtypes,and PACI type has more severe atherosclerosis. |
