Validity Of Padua And Caprini Risk Assessment Models In Identifying Venous Thromboembolism Risk Patients Among Hospitalized Patients

Objective To evaluate and compare Caprini risk assessment model(Caprini RAM)and Padua risk assessment model(Padua RAM)in identifying venous thromboembolism risk patients among hospitalized patients.Methods A retrospective study was conducted to collect patients admitted to general hospital of Ningxia university from January 2012 to December 2015.A total of 604 patients were difinited VTE during hospitalized,and 1260 patients without VTE according randomly selecting from admitted into the same departments and the same time.According to the Caprini RAM and Padua RAM,the relevant risk factors of the two groups of patients were scored at the same time,and the scores were compared.The relationship between risk classification and VTE occurrence in Caprini RAM and Padua RAM were discussed.The two risk assessment models of Caprini and Padua were compared to predict the effect of VTE in internal medicineinpatients.Result The average Caprini score(5.4 ± 3.3),the average Padua score(4.0 ±2.3),both were significant higher than that in controls(3.7±2.1?3.2±1.8,P<0.01).In Caprini RAM,low risk 0-1was used as the reference,and there was no difference between the risk of VTE at a low risk in moderate risk(2 points)and high risk(3-4 points,P > 0.05).Among the high-risk patients,the Caprini score of 5-6 VTE risk was 2.2-fold(P<0.01)than that at low-risk patients,and Caprini score of 7-8 VTE risk was 5.7-fold than that of 0-1 points,and Caprini score ? 9 points VTE risk was7.9-fold than that of 0-1 points(P<0.01),the higher the risk classification based on Caprini risk assessment model,the greater the risk of VTE.In the Padua model,taking the low risk 0-1 as the reference,there was no difference in the risk of VTE between low risk Padua score of 2-3 and high risk Padua score of 4-5(P>0.05).The Padua score of 6-7 VTE risk was 3.1-fold of that of 0-1(P<0.01),The Padua score > 8 VTE risk was 9.1-fold of that of 0-1(P<0.01),the higher the risk classification based on Pauda risk assessment model,the greater the risk of VTE.The AUC value of Caprini RAM(AUC=0.65),significantly higher than the value of the Padua RAM(AUC=0.59,P <0.05).The sensitivity and specificity of Caprini RAM were 67.5% and 50.8%,cutoff values was 3.A sensitivity of 37.7% and a specificity of 77.5% in Padua RAM.In this model,the recent(less than 1month)Trauma or surgery,acute infection and/or rheumatic disease,and being on hormone therapy are the high risk factors for VTE risk factors.Conclusion 1.Both Padua and Caprini risk assessment models can be used to predict the risk of VTE in medical inpatients.2.Caprini risk assessment model was better than Padua risk assessment model in predicting VTE in medical patients.
