Objective: Partial ureteral obstruction is a common disease in department of urology,leading to hydronephrosis,ureteral obstruction,but the disease process itself will precede the change of ureteral renal insufficiency,explore the change of smooth muscle structure and function of ureteral obstruction in different time,for how to control the clinical non operative treatment of ureter obstruction and the length of time ureteral theory irreversible damage to provide.Methods: 120 SD rats(male = 60)were randomly divided into negative control group,control group and experimental group;negative control group 30,control group and experimental group with 45 rats in each group,negative control group without intervention,the control group after anesthesia abdominal incision,free right ureter by about 2.5cm.Suture of abdominal incision;experimental group with psoas major muscle embedding underwent right lower ureteral embedding,establish ureter obstruction model;postoperative groups 1,2 and 3 weeks were randomly sacrificed rats,observation,photography and cut bilateral kidney and ureter,the negative control group were sacrificed10 week only,the experimental group and the control group were only 15 per week.The ureter above the obstruction area can be divided into two parts.Some of them are stored in-80 C refrigerator,and the other part is made by paraformaldehyde 4%.By observing and measuring ureteral diameter at different time intervals,the expression of myosin in ureteral smooth muscle was qualitatively detected by immunohistochemistry.RT-PCR was used to detect the expression of mRNA in smooth muscle myosin heavy chain.Results: 1.Morphological observation by naked eye: the ureter negative control group and the control group had no right ureteral obstruction,adhesions,no obvious thickening,the experimental group 1,2,3 weeks right ureter embedded at adhesion,encapsulation ofproximal ureteral thickening,and no obvious difference on the distal side of the distal cut see urine overflow,obstruction 3 weeks of ureteral thickening obviously,kidney volume also increased significantly;the experimental group saw open resection of ureteral lumen wider than the control group;microscope observation: negative control group and control group each week ureteral wall had no obvious change,slight inflammatory reaction,a small amount of inflammatory cell infiltration;1 weeks in the experimental group: right ureteral wall thickening part,smooth muscle hyperplasia,connective tissue hyperplasia,a small amount of visible fiber;2 weeks: right ureteral wall thickening,thickening of smooth muscle cells,proliferation,nuclear enlargement,some cells arranged in disorder Ureteral fibrosis was seen in 3 weeks: the right side ureter wall became thinner,the transverse section smooth muscle decreased,the nucleus became smaller,the muscle cells decreased,and a large number of fibrosis and connective tissue hyperplasia.2.Ureter diameter changes in the negative control group,the control group and the experimental group are different for each week of the right ureter diameter between the group and the negative control group right ureter diameter had no significant difference compared to control(P > 0.05),and two experimental groups each week had significant difference(P < 0.01);the experimental group with obstruction time prolonged,obstruction of the proximal ureteral dilatation gradually increased and the diameter gradually increased to third week group the most significant differences compared with the other groups was significant(P < 0.01).3.Ureteral smooth muscle myosin expression in experiment group right ureteral smooth muscle myosin in varying degrees,compared with the negative control showed no significant difference(P > 0.05),there was significant difference between the two and the experimental group(P < 0.01);the experimental group 1 and 2 weeks on the right side of obstruction of the proximal ureteral smooth muscle myosin expression gradually the most obvious increase,2 weeks,3 weeks was significantly decreased,there was significant difference(P < 0.01),and 3 week group was lower than that of the negative control group and the control group(P < 0.01).4.Ureteral smooth muscle myosin heavy chain mRNA expression in the negative control group,the control group and the experimental group each week all have different degrees of expression,there is no significant difference between negative control group and control group differences(P > 0.05),there was significant two and the experimental group(P < 0.01 weeks difference);experimental group 1 and 2 weeks on the right side of obstruction of the proximal ureteral smooth muscle myosin heavy chain mRNA expression was significantly higher than the negative control and the control group(P < 0.01);3weeks on the right side of obstruction of the proximal ureteral smooth muscle myosin heavy chain mRNA expression was significantly lower than the negative control group and control group(P < 0.01);the experimental group was 1,2 and 3 weeks of myosin heavy chain mRNA expression with prolonged obstruction increased first and then decreased,1,2weeks,2 weeks to 3 weeks was reduced,significantly lower than the 1 weeks group(P <0.01)?Conclusion:1.The longer the ureteral insufficiency,the more serious the ureteral smooth muscle damage.2.The changes of myosin and its heavy chain expression in the smooth muscle cells were basically in accordance with the changes of ureteral morphologic structure during the continuous process of ureteral insufficiency.3.More than 2 weeks of ureteral obstruction,the changes of morphological structure and molecular level were obvious.
