Objective: To investigate the relationship between serum non-HDL-C and acute ST-elevation myocardium by observing non-high-density lipoprotein in patients with acute ST segment elevation myocardial infarction at the First Affiliated Hospital of Shihezi University Correlation of Coronary Artery Lesions in Infarct Patients. Methods: A retrospective analysis of 276 patients who were diagnosed as having acute ST-segment elevation myocardial infarction by coronary angiography from January 2016 to July 2017 in the vascular diagnosis and treatment center of the First Affiliated Hospital of Shihezi University was performed. All patients underwent coronary angiography.According to coronary angiogram results, they were divided into three groups:single-vessel disease,double-vessel disease, and triple-vessel disease.Patients with coronary artery stenosis were calculated according to the Gensini score system. The scores of coronary artery lesions were divided into three groups: low-risk group(<41 points),moderate-risk group(41?Gensini score<64),and high-risk group(?64 points). Each group recorded general information at the time of admission(including gender, age, smoking history, drinking history, history of hypertension, diabetes history, type of living,height,weight),laboratory-related tests(including total cholesterol, triglycerides, and low Density of lipoprotein cholesterol,high-density lipoprotein cholesterol, etc.)and calculate non-HDL-C(non-HDL-C=TC-HDL-C),BMI(BMI = weight/height 2),the clinical efficacy of the above two groups The data were statistically analyzed and compared. Finally, multivariate logistic multiple regression was used for statistical analysis to further screen independent risk factors for coronary artery disease severity. All of these were performed using SPSS 17.0 software. Results: 1.A total of 276 patients with STEMI were enrolled in this study,of which 138 cases were single vessel disease group,89 cases were double vessel lesion group,49 cases were three vessel lesion group. The total serum cholesterol, low density lipoprotein cholesterol, non high density lipoprotein cholesterol and the living types of the three groups were statistically different(P<0.05),and there were no statistical differences in age, sex,BMI and high density lipoprotein cholesterol(P > 0.05).2.With the dependent variable of coronary artery disease in STEMI patients,an orderly Logistic regression analysis was performed on the risk factors of low density lipoprotein cholesterol, non high density lipoprotein cholesterol and total cholesterol. The results showed that non-HDL-C entered the equation, which was a risk factor for multiple coronary lesions(0.003,P value was <0.05).3.According to the Gensini score of 276 cases of STEMI enrolled in this study,68 patients in the high score group,65 in the middle group, and 143 in the low group.Compared with the general clinical data between groups of high, middle and low groups, there were significant differences in serum total cholesterol, low density lipoprotein cholesterol and non high density lipoprotein cholesterol in the three groups(all P<0.05);age,sex,BMI,type of residence and high density lipoprotein cholesterol were not statistically different(all P > 0.05).4.The Gensini score of STEMI patients was the dependent variable. The result of the sequential Logistic regression analysis with the risk factors of low density lipoprotein cholesterol, non high density lipoprotein cholesterol and total cholesterol showed that non-HDL-C entered the equation, which was the risk factor of the Gensini score,(the value of beta was 1.621,and the P value was <0.05).5.The correlation analysis between non-HDL-C and the severity of coronary artery disease(Gensini score)showed that there was a linear correlation between non-HDL-C and Gensini score(r=0.306,P=0.003).Conclusion: 1,High level of non-HDL-C is a risk factor for STEMI patients with multiple coronary lesions and aggravated lesions.2.Non-HDL-C is associated with the severity of coronary artery disease and STEMI.It can be used as an indicator of lipid management in CHD patients.
