Objective: To evaluate the clinical value of lymphography in the diagnosis of pelvic lymph node metastasis in cervical cancer. Methods: Relevant studies were identified by systematically searching Pubmed,Embase,Cochrane,CBM,CNKI,Wanfang,VIP.Retrieval time: update to December31,2017.Search scope: including all studies of lymphography in diagnosis of cervical cancer and pelvic lymph node metastasis. All studies were independently reviewed and selected by two evaluators.Quality Assessment of Diagnostic Accuracy Studies(QUADAS-2)items were used to evaluate the quality of included studies.Meta-Disc1.4 software was used to handle data for included studies, and then evaluate the clinical value of lymphography in the diagnosis of pelvic lymph node metastasis in cervical cancer. Results: A total of 19 studies were included, including 13 studies for English and 6studies for Chinese, and 1272 patients were included.Meta analysis showed:the sensitivity and specificity for lymphography in the diagnosis of cervical cancer pelvic lymph node metastasis were 65%(95%CI:59%-70%)and 84%(95%CI:81%-86%),AUC=0.7688,Q*=0.7091.Traditional lymphography and Magnetic resonance lymphography diagnosis of pelvic lymph node metastasis of cervical cancer of the sensitivity were 62%(95%CI:56%-68%)and 80%(95%CI:66%-91%),the specificity were 79%(95%CI:76%-82%)and 100%(95%CI:98%-100%),AUCLAG=0.7740,AUCMRL=0.8045;Q*LAG=0.7135,Q*MRL=0.7398.Conclusion: Lymphography has some significance in diagnosis of cervical cancer pelvic lymphatic metastasis, but it is not accurate. The clinical value of magnetic resonance indirect lymphography in the diagnosis of pelvic lymph node metastasis of cervical cancer is better than that of traditional lymphography.
