Objective:To study the clinical characteristics of hyperechoic Parkinson's disease with transcranial ultrasound and explore the influencing factors of hyperechoic substantia nigra.Methods:1.From May 2015 to July 2017,134 patients with Parkinson's disease diagnosed as Parkinson's disease according to the UK brain bank diagnostic criteria were selected.General data of the patients were collected:sex,age,onset age.The course of the disease was evaluated by UPDRS-III MOCA score.2.At the same time,TCS evaluation is carried out,and semi-quantitative classification is carried out according to the black echo intensity,and the black echo area of the patients with the black echo intensity above the grade III is measured,and the intensity of the echo is divided into normal echo(single-sided echo area?0.20 cm~2,SN+)and high echo according to the area size(single-sided echo area?0.20 cm~2,SN-).3.According to the intensity of substantia nigra,the patients were divided into two groups:SN+group and SN-group.The sex,age,onset age and course of disease were different between the two groups.4.According to the score of MOCA,the patients were divided into two groups:cognitive impairment group(MOCA<26)and non-cognitive disorder group(MOCA?26),comparing with SN+and SN-.5.According to the ratio of tremor score to ankylosis score,Parkinson's patients were divided into three groups:PIGD,TD and indeterminate.The differences of general data and area of substantia nigra were compared among these groups.Results:(1 Among 134 patients with Parkinson's disease,114 cases(85.1%)showed SN+,and the morbidity of male patients in SN+group was higher than that in SN-group(P=0.014),the onset age of SN+group was higher than that of SN-group(P=0.013).(2)According to the onset age of 60 years,the subjects were divided into two groups:the indeterminatedle-age onset group and the geriatric-age group.The group of SN+was mainly geriatric onset group.(3)In SN+group,there were more patients with cognitive impairment than that in SN-group(?~2=4.456,P=0.035)(4)No obvious distribution was found between the type of symptoms of Parkinson's disease and the results of sonographic correlation in the substantia nigra.Conclusion:The positive rate of SN was higher in older male patients.In PD patients with SN,the positive rate of SN was higher.The motor symptoms of Parkinson's disease with ankylosis as the main symptom were more serious.The type of symptoms of Parkinson's disease and the area of substantia nigra had not found obvious distribution characteristics.
