Objective: To evaluate the factors associated with substantia nigra hyperecho in patients with PD.Methods: 73 PD patients hospitalized in the department of neurology of Hebei General hospital from October 2017 to January 2020 were studied.According to the results of transcranial ultrasound,the patients were divided into PDSN+ group with abnormal echoes of substantia nigra and PDSNgroup with normal echoes of substantia nigra.The datas of age,age of onset,gender,the form of onset,course of disease,H-Y grades,Red blood cell count(RBC),hemoglobin(HGB),Homocysteine(HCY),Uric acid(UA)and Glomerular filtration rate(GFR)were recorded.To explore the differences between the two groups of patients in the above factors;Then PDSN+ group was selected to analyze the risk factors of substantia nigra hyperecho and the related factors of the size of high echo.Results:1.The average age of patients in the PDSN+ group was 70.31±7.78 years old,and that in the PDSN-group was 67.24±8.90 years old.The difference between the two groups was statistically significant(P > 0.05).The mean age of onset of PDSN+ group and PDSN-group was 65.35±9.48 years old and 64.34±9.35 years old.There was no significant difference in the age of onset between the two groups(P > 0.05).There was no significant difference in the course of 3.00(4.00)and 2.00(4.00)between the two groups(P > 0.05).There were 48 cases in the PDSN+ group,including 29 males and 19 females.There were 25 cases in PDSN-group,including 13 males and 12 females.There was no significant gender difference between the two groups(P > 0.05).In the PDSN+ group,there were 32 cases of patients with the form of tremor dominant(TD),and 16 cases of non-tremor dominant(TD).In PDSN-group,there were 18 patients with TD and 7 patients with NTD.There was no significant difference in the form of onset between the two groups(P > 0.05).2.In the PDSN+ group,there were 17 patients with H-Y grade ?3,and 31 patients with H-Y grade <3.In PDSN-group,there were 3 patients with H-Y grade ?3,and 22 patients with H-Y grade <3.The difference of H-Y grades between the two groups was statistically significant(P < 0.05).3.There was no significant difference in RBC,HGB,HCY,UA or GFR between the PDSN+ group and the PDSN-group(P > 0.05).4.The area of substantia nigra hyperecho was correlated with the echo level of substantia nigra and H-Y grades(P < 0.05).And the value of Spearman correlation coefficients were 0.435 and 0.417.The area of substantia nigra hyperecho was not correlated with age,age of onset,gender,onset form,course of disease,RBC,HGB,HCY,UA and GFR of PD patients.Conclusion:1.The high echo of the transcranial substantia nigra is correlated with H-Y grades in PD patients.The higher the H-Y grade,the more likely the patients are to have a high echo of the substantia nigra.2.The area of substantia nigra hyperecho is correlated with the level of substantia nigra echo and H-Y grade.The higher echo of substantia nigra and H-Y grade,the larger area of high echo of substantia nigra.
