Objective: To investigate the clinical effect of two different surgical procedures of cervical anterior cervical corpectomy and fusion with posterior single open-door laminoplasty in the treatment of multi-segmental cervical spondylotic myelopathy.Methods: A total of 60 patients with multi-segmental cervical spondylotic myelopathy from January 2014 to December 2016 were enrolled in this study.30 patients received cervical anterior cervical corpectomy and fusion(anterior group),30 patients received posterior single open-door laminoplasty(posterior group).The clinical results of the two groups were evaluated:operation time,intraoperative blood loss,hospitalization time and complication rate.The clinical outcomes were analyzed by JOA scores,preoperative,postoperative 1 week,postoperative 12 weeks and postoperative 24 weeks.Results: There were no significant differences in hospital stay and complication rate between the two groups,but the posterior group’s operation time was lesser than anterior group,there was significant difference between the two groups(P<0.05).The anterio JOA scores was(9.27±2.05)before operation and(18.63±1.87)after operation 1 week,(21.50±1.38)after operation 12 weeks and(23.63±1.27)after operation 24 weeks,the posterior group JOA scores was(9.43±2.46)before operation and(17.80±1.65)after operation 1 week(20.80 ± 1.58)after operation 12 weeks and(22.97 ± 1.77)after operation 24 weeks.The results of JOA scores showed that the scores of JOA in the two groups were significantly improved compared with those before operation and the difference was statistically significant.There was no significant difference in the JOA scores between the two groups at the first week,the twelfth weeks and the twenty-fourth weeks.Conclusion: Anterior approach and posterior approach have the same effect for the neurological function recovery in the treatment of multilevel cervical spondylotic myelopathy,posterior procedure operation time is less than anterior approach.
