Study On The Changes Of Thrombelastography Parameters And Inflammatory Factors In Patients With Type 2 Diabetes Mellitus Complicated With Acute Cerebral Infarction

Objective: The changes of serum inflammatory factors such as high sensitivity C-reactive protein(hs-CRP),interleukin-6(IL-6),soluble P-selectin,and thrombelastography(TEG)in patients with type 2 diabetes combined with acute cerebral infarction were discussed.In order to provide help for prevention and treatment of type2 diabetes mellitus complicated with acute cerebral infarction.Methods: A total of 94 patients treated in zibo first hospital,department of Endocrinology and Neurology,31 cases of patients with type 2 diabetes mellitus complicated with acute cerebral infarction were group I;31 cases of patients with pure acute cerebral infarction(no diabetes)were group II;32 cases of type 2 diabetes without obvious complications in patients of group III.In addition,32 cases of physical examination in same hospital during the same period were selected as the group IV.The living environment of each group was similar,age and sex did not have statistical significance,and there was a comparability between each group.There was no difference in the history of diabetes between group I and group III.The thrombelastography parameters and serum levels of the high sensitivity C reactive protein,interleukin-6,soluble P-selectin in each group were detected and the statistical analysis was carried out with SPSS19.0 statistics software.The research plan was approved by the academic committee of the hospital and the ethics committee.During the research process,the informed consent of the patients or family members was obtained,and the informed consent was signed.Results:(1)The levels of serum high sensitivity C reactive protein and soluble P-selectin showed that group I,group II and group III were significantly higher than those in group IV,and the difference was statistically significant(P<0.001,P<0.01,P<0.05).Serum interleukin-6 levels in group I and group II were significantly higher than those in group IV,with statistically significant difference(P<0.001,P<0.01).However,there was no significant difference between group III and group IV(P>0.05).(2)The TEG parameters of R,K,MA and CI showed that the difference between group I and group IV was statistically significant(P<0.01).The difference of TEG parameters of Angle between group I and group III was statistically significant(P<0.05).the same as group IV.The R value of group I was statistically significant compared with group III(P<0.05).(3)The TEG parameters of R,K,Angle,MA and CI showed that the difference between group II and group IV was statistically significant(P<0.05).The difference of R and Angle between group II and group III was statistically significant(P<0.05).(4)The TEG parameters of K and MA in group III were statistically significant compared with those in group IV(P<0.05).(5)The TEG parameters in group I were positively correlated with high sensitive C reactive protein,interleukin-6,soluble P-selectin(r=0.389,0.375,0.468;P=0.036,0.044,026).the TEG parameters in group II were also positively correlated with high sensitive C reactive protein,interleukin 6,soluble P-selectin,but the TEG parameters in group III,there was no significant correlation between the parameters and hypersensitivity C reactive protein,interleukin 6 and soluble P-selectin.Conclusion:(1)The elevated levels of serum hypersensitive C reactive protein,interleukin-6 and soluble P-selectin is correlated with the occurrence and development of type 2 diabetes with acute cerebral infarction.(2)The TEG parameters,especially the increase of MA value,may indicate the probability of thrombosis.
