Objective:To study the relationship between serum homocysteine?Hcy?and bone density?bone mineral density,BMD?in elderly women,to know the effect of HCY on bone mineral density in elderly women.Methods:1.we collected 201 cases elderly women aged above 60 years from our hospital in September2016 to Dec 2017.Using the American GE company's dual energy X-ray detector to determine the participants?L1L4?of the lumbar spine,femoral neck?FN?and total hip?TH?of bone mineral density.According to the results of dual energy X-ray BMD examination,subjects were divided into normal bone mass group?n=42?,osteopenia group?n=98?and osteoporosis group?n=61?.2.The age,menopausal age,menopause duration,height and weight were recorded ang measured,then calculated the bady mass index?BMI?.The level of blood calcium?Ca?,phosphorus?P?,alkaline phosphatase?ALP?,triglycerides?TG?,low density lipoprotein cholesterol?LDL-C?,high-density lipoprotein cholesterol?HDL-C?and albumin were detected by Roche Diagnosis Ltd production of fully automatic biochemical analyzer.Hcy was measured using enzyme linked immunosorbent assay?ELISA?.3.The data was analysed by SPSS20.0 including one-way ANOVA,Pearson correlation analysis,multi-factor Logistic Regression Method.P<0.05 for the difference was statistically significant.Results:1.Comparison of data between the three groups:42 cases of bone mass normal group,the averageageof67.18±3.31,osteopeniagroup98cases,theaverageageof67.84±3.28,osteoporosis group 61 cases,the average age of 67.98±3.36.There was no statistical difference in three group's age?P>0.05?.2.Comparison of the menstrual history of the three groups:the premenopausal age of the OP group and the osteopenia group was earlier than that in the normal group?46.62±2.84?46.89±3.94?49.48±2.46,P<0.01?.The menopause duration of OP group and osteopenia group was longerr than the normal group?20.79±3.24?20.14±2.53?18.71±2.26,P<0.01?.3.The gestation and chikdbirth history of the three groups:the OP and osteopenia group's gestation?4.64±1.59?4.57±1.36?3.76±1.20?and chikdbirth?4.23±1.45?4.03±1.24?3.40±0.96?was more than normal group,the difference was statistically significant?P<0.01?.4.The BMI of the three groups:BMI in the OP group and the osteopenia group was laeger than that in the normal group?19.46±1.99?20.96±2.46?22.26±2.09,P<0.01?.5.The Hcy and general biochemical indicators of the three groups:Albumin,HDL-C,Ca,P and ALP were not statistically significant?P>0.05?.The level of HCY in OP group and osteopenia group was higher than that in the normal group?10.78±3.13?17.44±3.06?26.25±4.37,P<0.01?.Compared with the normal group,the level of LDL-C in the OP group and the osteopenia group is increased?2.54±0.70?3.22±0.90?3.22±1.03,P<0.01?.The TG level of the normal group was higher than that of the osteopenia group and the OP group?1.53±0.60?1.41±0.49?1.27±0.42?,but there was no statistically significant difference between the normal group and the osteopenia group.6.Pearson correlation analysis showed that there was no correlation between the age of menopause,albumin,HDL-C,Ca,P,ALP and L1-4,femoral neck,and total hip bone density.Age?r=0.222,r=0.268,r=0.279,P<0.05?,age of menopause?r=0.431,r=0.309,r=0.411,P<0.01?were positively correlated with the bone density of L1-4,femoral neck and total hip.The bone density of L1-4 was positively correlated with TG?r=0.139,P<0.05?and BMI?r=0.311,P<0.01?.The bone density and Hcy?r=-0.531,r=-0.351,r=-0.324,P<0.01?,and LDL-C?r=-0.181,r=-0.212,r=-0.159,r=-0.159,P<0.05?were negatively correlated.At the same time,the bone density and the pregnancy rate?r=-0.224,P<0.01?and the childbirth?r=-0.207,P<0.01?of L1 and 4 were negatively correlated.7.Logistic regression analysis showed that:with bone mineral density decreased as the dependent variable,in menopause age(X1),menopause duration?X2?,progesterone?X3?,production time?X4?and BMI?X5?,HCY?X6?,TG?X7?,LDL-C?X8?as the independent variables,multivariate linear regression analysis,the results show that the elderly women menopause age,menopause duration,BMI,gestational,childbirth,Hcy and LDL-C into regression equation.Regression equation:y=-2.714-0.558X1+0.564X2-2.299X3-2.037X4-1.95X5+1.312 X6+1.25 X8Conclusion:1.The level of Hcy is significantly negatively correlated with bone density,and the level of Hcy can be used as a reference for evaluating the loss of bone mass in elderly women.2.Risk factors affecting the bone density of elderly women include:menopause duration?OR=1.758?,gestational?OR=9.962?,childbirth?OR=0.13?,HCY?OR=3.715?,and LDL-C?OR=3.491?.Protective factors affecting the bone density of elderly women include:menopause age?OR=0.572?and BMI?OR=0.823?;3.The level of LDL-C is significantly negatively correlated with bone density.
