Objective:(1)To explore the value of CT texture analysis in identifying benign and malignant solitary pulmonary nodules in plain scan.(2)To explore the value of CT texture analysis in identifying contrast-enhanced benign and malignant solitary pulmonary nodules.Methods:(1)Ninety-four patients with malignant solitary pulmonary nodules(54 malignant and 43 benign)were collected retrospectively from May 2014 to May 2017 in our hospital.Thoracic scans were performed with dual-source CT and 128-slice CT,Size and average CT value of the solitary pulmonary nodules, and then use the MaZda software CT texture analysis of solitary pulmonary nodules, get the texture parameters(entropy,Entropy SumEntrp,Entropy Difference,Contrast,Correlat,SumAverg). The independent samples t test was used to measure the parameters of benign and malignant nodules. The receiver operating characteristic curve(ROC)was established and the area under the curve(AUC)was calculated.Specificity and sensitivity of malignant diagnosis.(2)Retrospectively collected 43 cases of solitary pulmonary nodules(26 cases of malignant,17 cases of benign)in our hospital from May 2014 to May 2017,The texture parameter entropy, entropy and entropy difference were measured. The independent samples t test were used to compare the parameters of benign and malignant parameters.Results:(1)The average diameter of benign nodules was 2.26 ± 0.57(1.00-2.97)cm,and the average diameter of malignant nodules was 2.52 ± 0.25(1.02-2.99)cm.There was no significant difference in mean diameter of nodules between the two groups(p = 0.079)The average CT value was 26.96 ± 9.92 HU,the average CT value of malignant nodules was 22.13 ± 8.55 HU,and the average CT value of malignant nodules and benign nodules was not statistically significant(p = 0.087).The contrast of texture parameters of benign and malignant nodules,(P> 0.05).The differences of entropy, entropy and entropy difference were statistically significant(p <0.05),the AUC were 0.758,0.738 and 0.734 respectively, and the sensitivity was 0.727,0.884,0.636 and specificity of 0.862,0.667,0.714 respectively. The diagnostic value of the three methods was 0.848 for AUC,0.905 for sensitivity and 0.727 for specificity.(2)There was no significant difference in the entropy, entropy and entropy difference between benign and malignant nodules in enhanced arterial phase and venous phase(p> 0.05).Conclusion:CT texture analysis can reflect the early heterogeneity of the lesion not easily found in the naked eye,and benign and malignant pulmonary nodules CT scan texture analysis showed significant statistical differences, combined with the morphological findings of solitary pulmonary nodules,The sensitivity and specificity of benign and malignant diagnosis can be further improved on the basis of morphological diagnosis. There is no statistically significant difference between enhanced and benign nodules in enhanced scan, but enhanced nodules in enhanced scan can provide other More information, combined with plain-text analysis, can further improve its benign and malignant...
