Mechanism Of Electro-acupuncture On Primary-yuan Point And Connecting-luo Point Alleviating Myocardium Injury In Rats With Acute Pancreatitis

Abstract/Summary:
Background:Pancreatic-cardiac syndrome(PCS)is a common complication of acute pancreatitis(AP),which refers to a series of precordial discomfort,such as shortness of breath,chest tightness,palpitations and other cardiovascular system disorder.A large number of clinical reports have already confirmed that electro-acupuncture can inhibit the inflammatory reaction,reduce myocardial enzyme levels,protect the heart,regulate heart rhythm and then improve the prognosis of patients with AP.But the efficacy and mechanism of using electro-acupuncture to treat PCS complicated with AP is still unclear.Objects:The main purpose of this experiment is to study the effect and mechanism of acupuncture,which is used to treat AP combined with myocardial damage based on the traditional Chinese medicine theory of "the heart and the small intestine are interior-exteriorly related.Methods:Twenty-four male Sprague-Dawley rats were randomly divided into 3 groups(n=8):sham operation group,model group and the electro-acupuncture group.In model group and electro-acupuncture group,3.5% sodium taurocholate were injected slowly via pancreatic biliary duct by micro-pump to induce AP model.After 2 hours of successful operation,electro-acupuncture treatment was performed(disperse-dense wave,frequency: 2/100Hz).The acupoints of Shenmen(HT7)and Tongli(HT5)were bilaterally selected at the same time.The electro-acupuncture was performed every 8 hours for 30 minutes and totally 3 times within 24 hours.The rats in sham operation group and the model group were put into a fixator-bottle instead of using acupuncture within the same time.After 36 h of modeling,the blood was taken at rats’ apex area(4ml each one).After the centrifugation,the serum was equivalently divided into 2 parts.One of them was sent to laboratory department for amylase(AMY),cardiac troponin I(c Tn I)and creatine kinase isoenzymes(CK-MB)detection.The other one was used to test the levels of tumor necrosis factor-α(TNF-α),interleukin 6(IL-6)and interleukin 10(IL-10).The pancreatic tissue,cardiac tissue and small intestine tissue were taken for pathologic observation.Results:Compared with sham operation group,the level of AMY(2736.56±609.56),CTn I(0.92±0.38),CK-MB(604.63±141.02)and the score of pancreatic tissue(3.26±0.65),cardiac tissue(2.84±0.32),small intestine tissue(2.81±0.54)in the model group increased significantly(p<0.05).After electro-acupuncture treatment,the level of AMY(2158.21±410.4),CTn I(0.47±0.17),CK-MB(288.00±59.26)and the score of pancreatic tissue(2.69±0.45),cardiac tissue(1.87±0.59),and small intestine tissue(2.10±0.44)in treatment group were reducing obviously(p<0.05).The model group got a high level of IL-6(79.69±30.21)and TNF-α(15.76±1.93)compared with the sham operation group(54.04±7.45,13.13±1.74,p<0.05).The treatment group had a lower level of IL-6(55.92±6.56)and a high level of IL-10(44.61±6.45)compared than those in the model group(p<0.05).While the level of IL-10(36.78±3.75)is similar in the model group compared with the sham operation group(p>0.05).The concentration change of TNF-α(14.08±1.18)was not obvious in the treatment group(p>0.05).Conclusion:Electro-acupuncture at primary-yuan and connecting-luo point of Shenmen(HT7)and Tongli(HT5)could help relieve the cardiac injury in rats with AP.The mechanism is possibly associated with raising anti-inflammatory IL-10 and reducing pro-inflammatory IL-6.
